进展期胃癌、大肠癌术后腹腔化疗的临床应用价值

目的　了解进展期胃癌、大肠癌 (含腹膜返折以上部位直肠癌 )术后腹腔内化疗对防治腹腔局部复发和转移的临床价值。方法　选择进展期胃癌、大肠癌 2 11例 ,分别随机收入全身化疗组 (单纯组 ) 10 6例及全身化疗 +腹腔化疗组 (综合组 ) 10 5例 ,两组全身化疗所使用药物、给药途径及治疗疗程相同 ,综合组术时置泵或置入硅胶管 ,部分病人采用腹腔穿刺置管方法给予腹腔内灌注化疗药物。结果　两组间在术后 3年生存率有显著差异 (P <0 0 1) ,毒性反应相似。结论　术后腹腔内化疗对进展期胃癌、大肠癌的复发转移有显著疗效 ,能有效提高 3年生存率 ,且该方法安全、易操作

The Clinical Value of Intraperitoneal Chemotherapy after the Operation of Advancing Gastric and Large Intestine Carcinoma

Objective To study the clinical effect of intraperitoneal chemotherapy after the operation of advancing gastric and large intestine carcinoma(AGLIC,including rectal carinoma above peritoneal reflection) on preventing intraperitoneal local relapse and metastasis.Methods Two hundred and eleven patients with AGLIC were selected, and randomly divided into 2 groups:the simple group:systemic chemotherapy( n =106) and the complex group:systemic chemotherapy in addition to intraperitoneal chemotherapy( n =105).The drugs routes and courses of administration of systemic chemotherapy in the two groups were simliar.Drug pumps or silica gel tubes were laid by abdominal paracentesis.The intraperitoneal chemotherapeutic drugs were used through those pumps or tubes. Results There was significant difference in the 3-year survival tate between the two groups after operation( P <0 01).The toxic reactions of the two groups were similar.Conclusions The intraperitoneal chemotherapy after operation in patients with AGLIC has obvious effect on preventing relapse and metastasis. It can effectively improve the 3-year survival rate.This method is safe and simple.
