妊娠合并急性阑尾炎的临床诊治分析

目的探讨妊娠合并急性阑尾炎的诊治方法。方法分析24例妊娠合并急性阑尾炎患者的临床资料。结果行保守治疗患者10例,其中治愈8例,2例因保守治疗无效转为手术治疗。行手术治疗患者共16例,其中10例行传统开腹手术,6例行腹腔镜阑尾切除术;术后10例出现先兆流产,经保胎治疗继续妊娠;术后流产1例,早产1例,其病理类型均为急性坏疽性阑尾炎。结论妊娠合并急性阑尾炎应尽早诊治,以确保母亲及胎儿安全。治疗中应根据患者病情和孕周,选择合适的治疗方式。

Analysis of the diagnosis and treatment for the pregnant patients with acute appendidtis

Objective To discuss the diagnosis and treatment for the pregnant patients with acute appendicitis. Methods Twenty-four pregnant patients with acute appendicitis were analyzed retrospectively. Results Ten patients were given conservative treatment in which 8 patients got cured,2 were operated because of the ineffectiveness of conservative treatment. Sixteen patients were given surgical treatments in which ten patients were treated with normal open abdomen surgery and 6 patients with appendicitis removal. Ten patients developed threatened abortion after the surgery got pregnant again after tocolytie theraphy, 1 patient was premature and 1 patient was in abortion after surgery. Both of them were acute gangrenous appendicitis. Conclusion Early diagnosis and treatment for the pregnant patients with acute appendicitis should be carried out so as to reduce the patients＇ risk. Choosing the treatment methods should he based on the patients＇ condition and gestational weeks.
