地佐辛与舒芬太尼应用于术后静脉镇痛的临床效果比较

目的:观察地佐辛、舒芬太尼用于乳腺癌根治手术术后静脉镇痛的效果及不良反应。方法:将60例ASAⅠ～Ⅱ择期行乳腺癌手术患者,随机分为地佐辛组(D组)、舒芬太尼(S组)两组,每组30例,术后行静脉自控镇痛(PCIA)。配方分别是D组:地佐辛0.8 mg/kg加盐酸昂丹司琼0.2 mg/kg加生理盐水稀释至100 ml;S组:舒芬太尼2.5μg/kg加盐酸昂丹司琼0.2 mg/kg加生理盐水稀释至100 ml。观察两组患者术后4 h、8 h、12 h、24 h、48 h视觉模拟评分(VAS)、镇静程度评分(Ramesy)、48 h内患者不良反应的情况。结果:两组配方均能为患者提供良好的术后镇痛,两组患者总的PCA按压次数差异无统计学意义(P>0.05)。D组患者Ramesy镇静评分及患者术后的总体满意度显著优于S组,差异有统计学意义(P﹤0.05),且不良反应少。结论:地佐辛用于乳腺癌根治手术PCIA,镇痛效果确切可靠且不良反应少,可用于临床。

Clinical efficiency of postoperative intravenous analgesia with dezocin e and sufentanil

Objective To evaluate the analgesia effects and side effects of PCIA with dezocine and suf entanil in radical surgery for breast cancer.Method Total 60 patients with ASAⅠ～Ⅱ that were scheduled fo r radi cal surgery for breast cancer under general anesthesia,randomly assigned into d ezocine group（Group D,n=30）,sufentanil group（Group S,n=30）.PCIA gradi e nts used for Group D and Group S was dezocine 0.8 mg/kg＋ Ondansetron HCl 0.2 m g /k g;sufentanil 2.5 μg/kg＋ Ondansetron HCl 0.2 mg/kg which were diluted to be 1 0 0 ml by adding saline.To observe the patient′s VAS,Ramesy scores and pressing ti me s of PCA equipment,side effects at the times at 4,8,12,24,48 h after operation. Results There was no signifi cant difference in the effects of the two kinds of PCIA.But group D showed bett e r outcomes regarding to Ranesy score and patient′s satisfactory degree than gro u p S.Group D also give less side effects.Conclusion Dezocine can take a satisfactory effect and fewer side effecs in PCIA in patients underwent radical surgery for breast cancer,and it ′s worth for clinical application.
