硬膜外恒速阵痛泵用于剖宫产术后的观察

目的　对硬膜外恒速镇痛泵输注吗啡进行剖宫产术后镇痛的观察。方法　随机选择 5 3例硬膜外麻醉下行子宫下段剖宫产术的产妇作为实验组 ,剖宫产结束时留置硬膜外导管 ,连接注有吗啡 5mg、氟哌啶5mg、0 75 %布比卡因 15ml加生理盐水至 10 0ml的恒速镇痛泵 ;5 2例作为对照组 ,采用度冷丁间断肌注镇痛。结果　 2组采用不同镇痛方法后 2h ,术后 4 8h内镇痛效果评定有显著性差异 (P <0 0 1)。 2组患者术后 2 4h内睡眠时间分别为 (5 0 3± 2 3)h和 (2 92± 1 6 8)h(P <0 0 1)。 2 4h内开始泌乳时间早 (P <0 0 5 ) ,每日母乳喂养次数明显增加 (P <0 0 5 )。 2组产妇呼吸、心率、血压无明显差异 (P >0 0 5 )。结论　硬膜外恒速镇痛泵用于剖宫产术后镇痛效果好 ,副反应小 ,能促进泌乳功能 ,增加母乳喂养次数 ,提高母乳喂养成功率。

Observation on the Effect of Epidural Using Morphine with Constant Speed Analgetic Pump after Cesarean Section

Objective To observe the effect of epidural using morphine with constant speed pump after cesarean section. Methods 53 cases of pregnant women who received cesarean section with epidural anesthesia as test group and 52 cases pregnant women were selected as control group. The conctant speed analgetic pump with 5mg morphine, 5mg fentanyl, 15ml of 75% bubicaine and 100ml sodium solution was used epidural after operation in test group and dolantin was injected intramuscularly and discontinuously in control group. Results There was significant difference between two groups in analgetic effect assessment at 2h and 48h after operation (P<0.01). The sleep time of two group were 5.03±2.3 and 2.92±1.68 (P<0 01). The time of initial secretion of milk was earlier within 24h (P<0.05) and feeding time was increased (P<0.05). There were no significant difference between two groups in breath, heart rate and blood pressure (P>0.05). Conclusion This method could obtain good analgeltic effect and less side effect. It could promote the secretion of milk, increase time and successful rate of feeding.
