血清IL-18、G-CSF的测定在胎膜早破中的临床意义

目的探讨孕妇血清中IL-18、G-CSF在胎膜早破合并亚临床绒毛膜羊膜炎的早期诊断中的临床意义。方法选择足月胎膜早破孕妇30例（实验组），同期年龄、孕周、孕次与实验组相近的足月胎膜未破孕妇30例（对照组），60例孕妇均没有临床感染表现。用酶联免疫吸附试验方法测定两组孕妇在分娩前血清中IL-18、G—CSF的水平，分娩对取胎膜组织做病理学检查。结果（1）实验组孕妇血清IL-18、G-CSF的水平明显高于对照组；（2）实验组孕妇亚临床绒毛膜羊膜炎的感染率明显高于对照组孕妇，且亚临床绒毛膜羊膜炎孕妇血清IL-18、G—CSF水平高于非绒毛膜羊膜炎者。结论孕妇血清IL-18、G—CSF水平的增高与亚临床绒毛膜炎有关。

Clinical Significance of Serum Levels of IL - 18, G - CSF in Diagnosing Premature Rupture of Membranes

Objective To study the clinical significance of IL 18, G - CSF in the early diagnosis of premature rupture of mem- branes combined with subclinical chorionic amniotic membrane inflammation. Methods 30 cases of full term gravidas diagnosed as PROM without any clinical signs were selected as the experimental group. Another 30 cases of normal pregnant women of similar gesta- tional age were selected as the control group. The serum levels of IL - 18 and G - CSF were detected by enzymel - inked immunosor- bent assay （ELISA） and the placenta were sent for pathological examination. Results The serum levels of IL - 18, G - CSF in the experimental group were significandy higher than those in the control group. The subclinical chorionic infection rate of premature rup- ture of membranes women was significantly higher than those in the control group. The levers of serum IL - 18, G - CSF in the women with histological chorioamnionitis were significandy higher than women without histological chorioamnionitis. Conclusion The in- creased serum levels of IL - 18 and G - CSF are helpful to diagnose PROM with chorioamnionitis at an earlier stage.
