颈前路减压植骨融合内固定术对颈椎曲度和椎间高度的影响

目的评价颈前路椎体次全切除髂骨植骨钢板内固定手术对颈椎生理曲度和椎间高度的影响。方法选择因脊髓型颈椎病而行椎体次全切除髂骨植骨钢板内固定手术的患者32例，通过术前和术后及随访期间的颈椎标准侧位片测量颈椎生理曲度“D”值和椎间高度，并对各组测量值进行统计学分析比较。结果术后即刻颈椎生理曲度“D”值平均增加6．17±2．53mm，椎间高度平均增加3．21±27mm，两者与术前比较差异均有显著性。32例均获得随访，随访时间2-3．2年。随访中颈椎生理曲度和椎间高度都有丢失，但丢失结果比较无统计学意义。结论颈前路椎体次全切除髂骨植骨钢板内固定能明显改善颈椎生理曲度和椎间高度，且术后随访期间能长期维持。

Influence of anterior cervical decompression and bone graft and internal fixation on cervical curve and intervertebral height

Objective To evaluate influence of anterior cervical decompression and bone graft and internal fixation on cervical curve and intervertebral height. Methods 32 patients with cervical spondylotie myelopathy were treated by anterior cervical decompression and bone graft and internal fixation,the ＂D＂value of cervical curve and intervertebral height before and after operation were measured and compared statistically,through the frontal and lateral X-ray film. Results All of 32 patients underwent follow-up,at least 2 years ＂D＂value was immediately increased by 6.17± 2.53mm averagely after operation,intervertebral height was increased by 3.21±1.27mm in average after operation.ln the follow-up,the cervical curve and intervertebral height were no significant dif- ference. Conclusion The cervical curve and intervertebral height can be affected significantly by anterior cervical decompression and bone graft and internal fixation and no significant difference dunng the follow-up.
