芪丹益肾降糖胶囊治疗2型糖尿病肾病的研究

目的评价芪丹益肾降糖胶囊治疗糖尿病肾病(DN)患者的临床疗效及对血液流变学的影响,并探讨其作用机制。方法将88例DN患者,采用双盲双模拟研究方法随机分为治疗组和对照组,从DN患者的中医临床症状积分、血液流变学、尿微量白蛋白、尿素氮、肌酐、血糖、血脂水平等方面进行前瞻性对比治疗研究。结果DN患者血脂紊乱、血液流变学指标异常。治疗后,2组患者的中医临床症状积分、血液流变学指标、尿微量白蛋白排泄率、尿素氮、肌酐、血糖、血脂水平明显降低(P<0.05及P<0.01);治疗组在总有效率、肾功能改善、血液流变学指标、血糖、血脂水平改善方面均优于对照组(P<0.05及P<0.01)。结论芪丹益肾降糖胶囊能明显改善DN患者的临床症状和体征;改善肾功能、血液流变学指标;调节血糖、血脂代谢,有效延缓和控制DN的自然进程,进而提高DN患者的生活质量。降低血液黏稠度、调节血糖、血脂代谢是其改善肾功能的部分作用机制。

Study of Qidanyishenjiangtang capsule on diabetic nephropathy

Objective It is to explore the curative effect of Yidanyishenjiangtang capsule on diabetic nephropathy(DN) and its effect on hemorheology,and to explore its mechanism.Methods 88 cases of patients with DN were randomly divided into treatment group and control group with double-blind and double-dummy design method.Then prospective and contrast study was done by analyze scores of clinical symptoms of Chinese medicine,hemorheology,microalbunminuria,carbamide,creatinine,blood sugar,blood lipids and so on.Results The patients'blood lipids were disorganized and hemorheological indexes were abnormal.After treatment,scores of clinical symptoms of Chinese medicine,and the levels of hemorheology,microalbunminuria,carbamide,creatinine,blood sugar,blood lipids and so on were all decreased obviously(P<0.05 or P<0.01) in both groups.The total effective rate and the improvement of kidney function,hemorheological indexes,blood sugar,blood liquid in treatment group were all better than that in control group(P<0.05 or P<0.01).Conclusion Qidanyishenjiangtang capsule could obviously improve the clinical symptoms and signs,kidney function and hemorhiological,accommodate blood sugar and lipid metabolism,and could effectively delay nature development of DN,so to improve the life quantity of patients with DN.Decreasing blood viscosity,accommodating metabolism of blood sugar and blood lipids may be part mechanism for improving kidney function.
