盐酸曲马多缓释片治疗慢性骨关节病和腰痛的疗效观察

目的评价盐酸曲马多缓释片治疗中度以上肌肉骨骼系统慢性持续性疼痛的临床疗效与安全性。方法已经诊断的1452名慢性骨关节病和腰背痛患者。男762例,女690例,平均年龄49.32岁（20～78岁）。患者服用盐酸曲马多缓释片4周。首剂50mg,2次/d。依病情调整剂量,最大剂量为400mg/d。患者每周到医院随访一次,并由医生填写临床观察表格。结果2、3、4周的失访率分别为2.6%、5.0%、7.4%。疼痛缓解率从第1周的70%升至第4周的94%,夜间疼痛发生率从治疗前的86%降到4周后的9%,睡眠质量良好率从治疗前的10%升至第4周的74%,能坚持原工作的患者从治疗前的31%升至第4周的58%,生活态度积极者从治疗前的13%升至第4周的51%,治疗顺应性从第1周的60%升至第4周的75%,本组出现的副作用为恶心8%、呕吐1%、眩晕2%。结论盐酸曲马多缓释片对治疗肌肉骨骼系统的慢性疼痛具有良好的疗效,其副作用轻微,耐受性较好。

A clinical survey on the analgesic effect of tramcontin on chronic osteoarthrosis and low back pain

Objective To evaluate the analgesic effect and safety of tramcontin in the treatment of moderately severe chronic musculoskeletal pain. Methods A total of 1452 patients including 762 men and 690 women with chronic osteoarthrosis and low back pain were involved, with the average age of 49.32 years (20-78 yrs). Tramcontin was given to all of them for 4 weeks with the initial dose of 50 mg/ Bid. Daily dose was determined by patient's response and the maximal dose was 400 mg/d. Patients were followed up once a week, and the clinical results were recorded by the doctorl. Results 1)Loss rate of follow-up was 2.6%, 5% and 7.4%, respectively at 2nd, 3rd, 4th week. 2) From 1st week to 4th week, pain relief rate was increased from 70% to 94%. Nocturnal pain rate was decreased from 86% to 9%. The ratio of good sleep increased from 10% to 74%, and the ratio of patients return to work increased from 31% to 58%. Furthermore, the ratio of patients with optimistic attitude increased from 13% to 51% and the patients' compliance was increased from 60% to 75%. 3) Adverse effects included nausea, vomiting and dizziness with the incidence of 8%, 1% and 2% respectively. Conclusion As a kind of retard tablets of Tramadol Hydrochloride, tramcontin has good compliance and a satisfactory analgesic effect on chronic musculoskeletal pain with moderate adverse reaction.
