神经导航在经蝶垂体瘤手术中的应用

摘    要:目的 探讨神经导航在经蝶垂体瘤术中的应用价值。方法 应用神经导航技术经蝶切除26例垂体腺瘤。术前行三维重建,确定手术入路,术中实时导航,监测肿瘤切除程度。结果 26例垂体瘤在神经导航引导下均能精确定位,肿瘤全切18例,次全切8例;术后7例出现一过性尿量增多,无脑脊液漏和颅内感染,无手术死亡。结论 经蝶垂体瘤手术中,神经导航可提供实时三维定位,有助于提高肿瘤切除率,减少手术并发症。

Abstract:Objective To evaluate the application value of neuronavigator-guided transsphenoidal microsurgery for the resection of pituitary adenomas.Methods 26 patients with pituitary adenomas underwent transsphenoidal microsurgery,before which three-dimension reconstruction was made,surgical approach was localized,and extent of lesion-resection was monitored during the operation.Results All the pituitary adenomas were localized accurately under the guidance of neuronavigator.Total tumor removal was achieved in 18 patients and subtotal removal in 8.Postoperatively transient diabetes insipidus occurred in 7 cases.There were no cerebrospinal fluid leakage,intracerebral infection and operative death.Conclusion During transsphenoidal microsurgery,neuronavigator provides real-time localization with three-dimensional information,which is quite helpful for increasing the total removal rate and reducing the surgical complications.
