心脏病患者室性心律失常Lown分级及室早指数分析

目的:探讨室性心律失常Lown分级、室早指数及昼夜分布情况对器质性心脏病患者的临床应用价值.方法:回顾性分析2004年1月～2011年12月常规心电图、动态心电图检出的540例室性心律失常患者的临床资料,比较器质性心脏病组(Ⅰ组,n=334)、非器质性心脏病组(Ⅱ组,n=206)室性心律失常Lown分级特点,两组间Lown分级Ⅲ级以上室性心律失常的室早指数及其昼夜分布特点.结果:Ⅰ组检出的Lown分级Ⅲ级以上室性心律失常159例(47.6%)显著高于Ⅱ组检出的30例(14.6%)(P＜0.01);Ⅰ组Lown分级Ⅲ级以上室性心律失常的室早指数为0.99±0.11,显著低于Ⅱ组的1.17±0.15(P＜0.01);Ⅰ组室性心律失常高峰出现在日间220例(66%),显著高于Ⅱ组的89例(43%)(P＜0.01).结论:对室性心律失常患者进行Lown分级、室早指数、室性心律失常高峰时间分布的综合分析,有助于区分室性心律失常的性质及提高对其预后的预测价值.

Analysis on Lown Classification of Ventricular Arrhythmia and Ventricular Premature Beat Index of Patient with Heart Disease

Objective： To explore the clinical application value of Lown classification of ventricular arrhythmia, premature ventrieular beat index and day/night distribution patterns of patients with organic heart-disease. Methods： The clinical data of 540 patients with ventrieular arrhythmia detected by electrocardiogram and dynamic electrocardiography were analyzed retrospectively from Jan 2004 to Dec 2011. The ventrieular arrhythmias in Lown grading characteristics of organic heart disease group （group I,n=334） and non-organic heart disease group （group Ⅱ ,n=206） were compared. The ventricular premature beat index over Lown grade m and day/night distribution patterns were also compared between the two groups. Results： Detection rate of ventricular arrhythmias above Lown grade Ⅲ in Group I was 159 （47.6%） while that in Group Ⅱ was 30 （4.6%） （P 〈 0.01）. The ventricular premature beat index above Lown grade Ⅱ in group I and group Ⅱ was 0.99±0.11 and 1.17±0.15 respectively （P 〈 0.01）. The diurnal cases of ventricular arrhythmias in group I and group Ⅱ were 220 （65.9%） and 89 （43.2%） respectively （P 〈 0.01）. Conclusion： With a comprehensive analysis on Lown elassifieation,ventricular premature beat index and fastigium distribution of ventricular arrhythmias, the nature of ventricular arrhythmias can be better distinguished, providing higher predictive value of prognosis for patient with ventricular arrhythmias.
