运用因子分析探讨急性冠脉综合征证候要素及靶位特征

目的探讨急性冠脉综合征(ACS)证候要素,为ACS辨证治疗提供依据。方法以315例ACS患者的临床资料为基础,运用频数分析、因子分析的方法,探索ACS的证候要素及其靶位特征。结果中医症状部分共提取16个公因子,根据文献研究以及专家组建议,将16个公因子归属为8个证候要素。本虚证候要素比例由高至低依次为:气虚>阳虚>阴虚,标实证候要素出现比例由高至低依次为:血瘀>痰饮>气滞>寒凝>热蕴。结论 ACS的证候要素是痰饮、血瘀、阳虚、气虚、气滞、寒凝、热蕴、阴虚;病性为本虚标实,病位在心,病变脏腑涉及肺、脾、肝、肾。

Syndrome elements and target characteristics of acute coronary syndrome investigated by applying factor analysis

Objective It is to study the syndrome elements of acute coronary syndrome(ACS),thus to provide for evidences for syndrome differentiation and treatment for this disease.Methods 315 ACS patients were selected and their clinical information of TCM was collected.Syndrome elements and target characteristics were extracted through frequency statistics and factor analysis.Results There were 16 common factors extracted by factor analysis.By literature search and expert panel proposal,8 types of TCM syndromes were found.Frequency of occurrences reduced in order of Qi deficiency,Yang deficiency,Yin deficiency among the syndromes of deficiency in origin and reduced in order of blood stasis,phlegm stagnancy,Qi stagnation,cold coagulation,accumulating heat among the syndromes of excess in superficiality.Conclusion The syndrome elements of ACS: phlegm stagnancy,blood stasis,Yang deficiency of,Qi deficiency,Qi stagnation,cold coagulation,accumulating heat,Yin deficiency.Characteristic of disease was asthenia in origin and excess in superficiality.Disease located in heart and covered lung,spleen,liver and kidney.
