ICAM-1、P选择素、nm-23在青年人结直肠癌中的表达及其临床意义

目的研究细胞间黏附分子（ICAM-1）、P选择素、nm-23在结直肠癌组织中的表达与临床病理和预后的关系。方法对92例青年人结直肠癌患者的手术切除标本、癌周正常大肠黏膜组织采用免疫组织化学方法检测其ICAM-1、P选择素、nm-23的表达情况，分析其变化规律及其相关性。结果92例结直肠癌患者中ICAM-1、P选择素阳性表达高于癌旁组织，在正常大肠粘膜组织中无表达（P〈0.05），nm-23在结直肠癌组织中表达则低于癌旁组织，三者表达与结直肠癌组织学类型、临床Dukes分期及转移复发有密切关系（P〈0.05），但与肿瘤大小和部位无关（P〉0.05）。结论结直肠癌组织中ICAM-1、P选择素的表达可能是肿瘤发生、浸润、转移的重要因素，和nm-23结合检测对判断肿瘤的恶性程度、转移情况、估计预后、评价复发及指导临床患者术后化疗具有重要的意义。

The expression of ICAM-1、P-selectin、nm-23 in human colorectal carcinoma and clinical study

Objective To observe the expression of ICAM-1,P-selectin and nm-23 in young colorectal carcinoma,along with investigation of its correlation with clinical pathology and prognosis. Methods Immuno- histochemical staining was explored to detect the resected specimens and normal mucosa around carcinoma of 92 young cases with colorectal carcinoma for the expression of ICAM-1, P selection and nm-23, analyzing the changes in the law and its relevance. Results Of 92 cases with colorectal carcinoma, the positive expression of ICAM-1 and P-selectin was higher than that of tissues near carcinoma, no expression in normal colorectal mucosa （P 〈 0. 05）, and expression of nm-23 in coloreetal cancer lower than that of tissues near carcinoma. The three expressions were not correlated with the size and site of the tumor （ P 〉 0. 05 ）, however, they were significantly correlated with histological types, Dukes clinical staging, and metastasis and recurrence of the colorectal carcinoma （P 〈 0.05 ）. Conclusion The expressions of ICAM-1 and P-selection in colorectal carcinoma may constitute the important factors involved in carcinogenesis development, infiltration and metastasis ; the detection along with nm-23 is of great significance in determining the stages of malignant tumors, metastasis, prognosis, and the clinical evaluation of patients with recurrent postoperative chemotherapy.
