Characteristics of blast injuries in modern warfare: a systematic review

Abstract:. Object Although the weapons evolve continuously, and combat styles changed thoroughly, in the past decade, explosive trauma have still domain the combat-related injuries requiring massive medical resource. Studying the frequency, types of injuries and characteristics of blast injuries in modern warfare is of great significance to clinic guideline, specific medical supplies, and protective equipment development. Method A systematic review were performed and refers to PRISMA to construct search strategies, literature inclusion/exclusion criteria, and statistical analysis. All literatures on the incidence, characteristic, mechanism, and special injury conditions of blast injuries were retrieved. Results A total of 31 articles were included in the analysis, consisting of 32,474 blast injury casualties came from the Iraq-Afghanistan war, the Cole attack incident and combat conflict in Croatia. The prevalence of blast injury was 40%-87.4%. The incidence of head, face and neck blast injuries was 15.8% -89.5%, the trunk blast injuries were 6.7% -80%, the limb blast injuries were 45.2% -96.6%, and the blast injuries in other parts were 15.4% -70.4%. According to the analysis of different injury mechanisms, impact injuries and penetrating injuries caused by explosions in open spaces account for the main part, while burns are the main injuries caused by explosions in closed spaces such as explosions on ship. The severity of secondary explosion injury represented by penetrating injury is the highest, and a large amount of injury control and surgical resuscitation forces need to be deployed. Conclusion This study provided evidences for the battlefield rescue and technology of our military through the analysis of the characteristics of blast injury in modern warfare.
