前置胎盘对新生儿出生体重的影响

目的:评估前置胎盘对胎儿出生体重的影响。方法:选取2010年5月～2011年8月间因前置胎盘于嘉兴市妇幼保健院住院分娩的68例患者为研究对象,依分娩孕周不同,依次分为孕28周+0天～31周+6天(A组)、孕32周+0天～33周+6天(B组)、孕34周+0天～36周+6天(C组)、孕37周+0天～40周+0天(D组)及孕40周+1天～41周+0天(E组)为实验组;同时取同时期同孕周单纯早产及足月妊娠的患者为对照组,按不同前置胎盘类型分类,比较相对应孕周出生时新生儿体重。结果:孕37周前新生儿体重与单纯早产组比较无统计学差异;孕37周～40周组新生儿体重与足月妊娠组相比无统计学差异;孕40周后前置胎盘新生儿体重较正常足月妊娠组偏高,且在前置胎盘合并胎盘植入患者中明显增高。结论:孕40周前前置胎盘对胎儿生长无影响,而孕40周后胎儿生长体重增高患者需警惕胎盘植入。

Effect of placental presentation on neonatal birth weight

Objective:To assess the effect of placental presentation on birth weight of fetuses.Methods:A total of 68 patients with placental presentation who gave birth to their babies in the hospital from May 2010 to August 2011 were selected as study objects,and they were divided into different experimental groups according to gestational weeks at delivery: 280+-316+ gestational weeks(group A),320+-336+ gestational weeks(group B),340+-366+ gestational weeks(group C),370+-400+ gestational weeks(group D),and 401+-410+ gestational weeks(group E);the patients with simple premature delivery and full-term pregnancy at the same gestational weeks were selected during the same period as control group,the patients were divided into different groups according to the types of placental presentation,the neonatal birth weights were compared among the neonates who were born at the same gestational weeks.Results:Before 37 gestational weeks,there was no statistically significant difference in neonatal birth weight between various experimental groups and simple premature delivery group;there was no statistically significant difference in neonatal birth weight between various experimental groups and full-term pregnancy group during 37-40 gestational weeks;the neonatal birth weights in various experimental groups after 40 gestational weeks were higher than that in full-term pregnancy group,and the neonatal birth weight in the patients with placental presentation combined with placental implantation increased significantly.Conclusion:Before 40 gestational weeks,placental presentation has no impact on fetal growth,but the patients with fetal body weight gain after 40 gestational weeks should be alert to placental presentation.
