上海浦东新区社区流动人口孕产妇全覆盖管理干预研究

目的:进一步完善现行的社区孕产妇系统管理模式,为政府制定流动人口孕产妇保健政策提供科学依据。方法:选取上海市浦东新区3个街道为干预社区,3个街道为对照社区,在干预社区实施孕产妇全覆盖管理干预模式提供孕产妇保健服务,在对照社区按常规提供孕产期保健服务。结果:随访2年后,试点社区流动人口孕产妇12周内建卡率为39.09%,对照组为33.72%,试点社区建卡率明显高于对照组(P=0.027);产前检查超过5次者占93.02%,超过对照组的88.16%(P<0.001)。经多因素非条件Logistic分析,干预后早孕建卡和孕检次数均有统计学差异。结论:全覆盖管理模式可以改善流动人口孕产妇管理现状,扩大孕产期保健的覆盖面。

Study on integrated coverage management and intervention of floating pregnant women in communities of Pudong New Area in Shanghai

Objective:To further perfect current system management mode of pregnant women in communities,provide scientific basis for the government to make maternal health care strategies for floating pregnant women.Methods:Three districts were selected from Pudong New Area in Shanghai as intervention communities,and three districts were selected as control communities,integrated coverage management and intervention mode was provided to the pregnant women in intervention communities,while conventional maternal health care service was provided to the pregnant women in control communities.Results:After follow-up for two years,the registration rates of floating pregnant women within 12 gestational weeks in intervention communities and control communities were 39.09% and 33.72%,respectively.The registration rate of floating pregnant women in intervention communities was statistically significantly higher than that in control communities(P=0.027).The proportion of floating pregnant women receiving prenatal examination for more than five times in intervention communities was 93.02%,which was statistically significantly higher than that in control communities(88.16%)(P<0.001).The results of non-conditional multivariate logistic analysis indicated that there was statistically significant difference in the registration rate of early pregnancy and the times of prenatal examination between before and after intervention.Conclusion:Integrated management mode can improve maternal management status among floating population and enlarge the coverage of maternal health care.
