博利康尼、布地奈德雾化吸入治疗婴幼儿毛细支气管炎的疗效观察

目的:观察博利康尼、布地奈德雾化吸入治疗婴幼儿毛细支气管炎的临床疗效。方法:两组婴幼儿毛细支气管炎患儿均给予常规治疗,有心力衰竭者加用强心、利尿、扩血管治疗。治疗组在此基础上加用博利康尼溶液1 ml/次(含硫酸特布他林2.5 mg)、布地奈德0.5～1 mg,每日2～3次雾化吸入15 min/次,疗程5～7天。观察两组的疗效、主要症状与体征的消失时间及肺功能指标。结果:①治疗组的痊愈率和总有效率均明显高于对照组(P<0.05)。②治疗组患儿的咳嗽、气促、喘息、肺部哮鸣音及肺部啰音等症状、体征消失时间均短于对照组(P<0.05)。③治疗后两组的潮气量、吸呼比、达峰时间比、达峰容积比等肺功能指标均较治疗前明显改善,且治疗组较对照组改善更明显(P<0.05)。④两组在治疗过程中均未见明显不良反应发生。结论:博利康尼、布地奈德雾化吸入治疗婴幼儿毛细支气管炎能明显改善临床症状、体征及肺功能,无明显不良反应,值得临床推广使用。

Observation on curative effect of aerosol inhalation of Bricanyl combined with Budesonide for treatment of infantile bronchiolitis

Objective:To observe the clinical curative effect of aerosol inhalation of Bricanyl combined with Budesonide for treatment of infantile bronchiolitis.Methods:The infants with bronchiolitis in control group and treatment group were treated with routine therapy,and the infants with heart failure were treated with cardiac agents,diuretic,and vasodilator drug therapy.Based on these treatments,the infants in treatment group were treated with aerosol inhalation of Bricanyl(1 ml per time,including 2.5 mg Terbutaline sulfate) combined with Budesonide(0.5-1 mg),15 minutes per time and 2-3 times per day,the course of treatment was 5-7 days.The curative effects,the disappearance times of main symptoms and signs,pulmonary function parameters in the two groups were observed.Results:The curative rate and total effective rate in treatment group were statistically significantly higher than those in control group(P<0.05).The disappearance times of main symptoms and signs,such as coughing,shortness of breath,wheezing,and lung rales,in treatment group were statistically significantly shorter than those in control group(P<0.05).The tidal volumes,inspiration and expiration ratios,the ratios of time to reach peak tidal expiratory flow to total expiratory time,and the ratios of volume to peak expiratory flow to total expiratory volume in the two groups after treatment were significantly better than those before treatment,and the improvements in treatment group were more obvious than those in control group(P<0.05).No obvious adverse reactions occurred in the two groups during the course of treatment.Conclusion:Aerosol inhalation of Bricanyl combined with Budesonide can improve clinical symptoms,signs,and pulmonary function obviously for treatment of infantile bronchiolitis,and no obvious adverse reactions occur,the method is worthy to be popularized and applied in clinic.
