Ⅰ期玻璃体切割术治疗后段眼内异物的临床疗效

目的评价采用I期玻璃体切割手术治疗眼后段异物的临床疗效。方法回顾性分析2008年6月-2012年1月,经I期玻璃体切割治疗眼后段异物72例,对手术前后视力、术后并发症及手术时机进行分析。结果 I期玻璃体切割异物摘出率为100.00%。术前62.50%患者视力〈0.1,术后视力≥0.1者56例（77.78%）,≥0.5者19例（26.39%）,术前术后视力比较差异有统计学意义（P〈0.05）。术后并发视网膜脱离3例,增生性玻璃体视网膜病变2例,玻璃体腔积血者1例;6例（8.33%）再次行玻璃体切割手术,术后视网膜均复位。结论 I期玻璃体切割手术治疗眼后段异物具有异物取出率高、术后视力恢复好、并发症少等优点。

Clinical curative effect of the first-stage vitrectomy for posterior segment foreign bodies

Objective To evaluate the clinical effects after extraction of posterior segment foreign bodies by first-stage vitrec- tomy. Methods From 2008 June to 2012 January,72 patients （72 eyes） with the diagnosis of IOFB following penetrating ocular were retrospectively evaluated. 72 eyes had undergone first-stage vitrectomy extraction. The relation of preoperation visual acuity, operation time to postoperative visual acuity, the complication of preoperation and postoperation were analyzed. Results Postoperatively, the suc- cessful rate of the extraction of foreign bodies in the first-stage vitrectomy group was 100%. Preoperation VA 〈 0.1 occurred in 45 of 72 （62.5%） in the first-stage vitrectomy group. After operation there ware 56 eyes（77.78% ） with the BCVA more than O. 1.19 eyes （26. 39% ）with the BCVA more than 0.5. The post operative BCVA was much better than that of preoperation（P 〈 O. 05）. There were 3 eyes with retinal detachment, 2 eyes with PVR, 1 eye with vitreous hemorrhage after the first-stage vitrectomy operation. 6 eyes （8.33 % ） had undergone the secondary vitrectomy, and after operation all retinal were attached. Conclusion The first-stage vitrectomy is an effective therapeutic approach in adequate and timely removing posterior segment intraocular foreign bodies, with the advantage of high extraction rate, better postoperative BCVA,lower operative and postoperative complication.
