乳腺癌新辅助化疗联合淋巴化疗对前哨淋巴结活检结果的影响

目的探讨乳腺癌新辅助联合淋巴化疗后前哨淋巴结活检的价值。方法 40例局部晚期乳腺癌患者随机分为试验组及对照组两组,分别进行新辅助联合淋巴化疗及仅进行新辅助化疗,对治疗后可切除病例(试验组治疗后可切除病例归入A组,对照组治疗后可切除病例归入B组)行前哨淋巴结活检、腋淋巴结清扫术,对活检资料及腋淋巴结情况进行分析。结果 A组患者检出率88.24%,准确性80%,假阴性率35.71%;B组检出率90.91%,准确性90%,假阴性率11.11%。两组准确性、假阴性率比较,差异均有统计学意义(P<0.05)。结论乳腺癌新辅助化疗联合淋巴化疗会干扰前哨淋巴结活检结果。

The effect of Neoadjuvant combined lymphatic chemotherapy on the result of sentinel lymph node biopsy of breast cancer

Objective To examine the value of sentinel lymph node biopsy after neoadjuvant combined lymphatic chemotherapy in breast cancer. Methods 40 cases of locally advanced breast cancer patients were randomly divided into two groups： 20 cases （trial group） received neoadjuvant combined lymphatic chemotherapy, and 20 cases （control group） only received neoadjuvant chemotherapy. After the treatment, the resectable cases （trial group after treatment of resect- able cases in group A, the control group after treatment of resectable cases in group B） underwent sentinel lymph node biopsy）had SLNB and axillary lymph node dissection （ALND）. The biopsy dates and axillary lymph node status were a- nalysis. Results In group A z the successful rates of SLNB were 88.24%, accuracy were 80%, false negative rates were 35.71%. Group B： The successful rates of SLNB were 90. 91%, accuracy were 90%, false negative rates were 11.11%. The accuracy and the false negative rate were significant different in two groups （P〈0. 05）. Conclusion Neoadjuvant combined lymphatic chemotherapy in breast cancer can interfere with the results of sentinel lymph node hionsv.
