| 摘 要： | 目的比较胸腔镜与腋下小切口治疗自发性气胸在微创及疗效方面的差异。方法对60例自发性气胸患者采用胸腔镜治疗(胸腔镜组),60例自发性气胸患者采用腋下小切口治疗(小切口组),比较两种方法在术中的出血量、手术时间、术后早期肺膨胀不全发生率方面的差异。结果胸腔镜组术中出血量平均48ml,手术时间平均51分钟,术后早期肺膨胀不全3例,发生率5%,与腋下小切口组比较,胸腔镜组的术中出血量、手术时间、术后早期肺膨胀不全发生率均显著低于腋下小切口组,差异有统计学意义(P<0.05)。结论胸腔镜治疗自发性气胸创伤小,出血少,手术时间短,术后恢复快,具有明确的微创疗效,可在临床推广应用。 |

| Abstract: | Objective Compare the differences of VATS and small incision at oxter to curing spontaneous pneumo- thorax. Methods Compare the bleeding amount, surgery time and atelectasis at the early time post-surgery between 60 cases who had VATS and 60 cases who had small incision at oxter. Results In VATS group, the average bleeding time is 48 ml, surgery time is 51 minutes, and 3 cases atelectasis, recurred, with the incident rate is 5%. The bleeding a- mount, surgery time and atelectasis rate at the early time post-surgery in the VATS group are obvious lower than that in small incision at oxter group（P〈0. 05）. Conclusion Obvious advantages such as smaller wound, less bleeding, shorter surgery time, and better recovery to cure spontaneous pnuemothorax by VATS. |
