CT引导肺穿刺活检对不同大小病灶的应用价值

目的:探讨CT引导肺穿刺活检对不同大小病灶的应用价值.材料和方法:回顾性分析CT引导肺部病变穿刺活检184例.病灶按大小分为两组,其中≥3cm者122个,<3cm者62个,分析穿刺活检术对两组病变的诊断正确率和并发症发生率.结果:CT引导肺穿刺活检对肺内大、小病灶的诊断正确率分别为92.6%(113/122)和91.9%(57/62;P>0.05).<3cm病灶的气胸发生率(19.2%)明显高于≥3cm病灶(5.2%;P<0.01),前者出血发生率(44.2%)亦显著高于后者(15.1%;P<0.01).结论:CT引导肺穿刺活检对大、小病灶的诊断准确率均较高,对大病灶活检的安全性高于小病灶.

The Value of CT-guided Percutaneous Transthoracic Lung Biopsy for Pulmonary Lesions with Different Size

Purpose: To evaluate the role of CT-guided percutaneous transthoracic lung biopsy for small and large pulmonary lesions. Materials and Methods: CT-guided percutaneous transthoracic lung biopsy was performed in 184 patients, all of which had surgical pathology or follow-up results. Of 184 lesions, 122 were larger than or equal to 3cm in diameter and 62 were lesser than 3cm in diameter. The diagnostic accuracy and complication rate were calculated in two groups and compared with each other. Results: The diagnostic accuracy for large lesions and small ones was 92.6%(113/122) and 91.9%(57/62) respectively, which had no significant difference (P>0.5). The pneumothorax rate for lesions smaller than 3cm (19.2%) was significantly higher than that for lesions larger than or equal to 3cm(5.2%; P<0.01).The bleeding rate for small lesions (44.2%) was also higher than that for large lesions(15.1%; P<0.01). Conclusion: CT-guided percutaneous transthoracic lung biopsy has a high diagnostic accuracy for both large and small pulmonary lesions. The operation for large lesions is safer than for small ones.
