尿毒症患者血清脂联素及其临床意义的研究

目的检测尿毒症患者血清脂联素水平,分析其对体内炎症反应的影响以及与肾脏功能、血红蛋白和白蛋白的关系。方法采用ELISA法测定60例尿毒症患者的血清脂联素(AD-PN)水平及血C-反应蛋白(CRP)、肿瘤坏死因子-a(TNF-a)、纤维蛋白原(Fbg),并分析它们之间的相互关系。结果在所有研究对象中有46.7%的患者CRP水平超过正常参考值5mg/L。实验组血ADPN、TNF-a和Fbg均高于对照组(P值均<0.05),单因素相关分析结果表明血ADPN与CRP、TNF-a和Fbg均呈明显负相关,相关系数分别为-0.49、-0.51和-0.43(P值均<0.01)。结论在尿毒症患者中,血清脂联素水平与体内炎症反应、贫血及营养不良密切相关,脂联素可能是一种保护性因子。

The clinical significance of serum adiponectin concentration in uremia patients

Objective To detect the serum adiponectin concentration in uremia patients and to analyze its influence on inflammatory reaction,as well as its relationship with renal function,hemoglobin and albumin.Method The plasma levels of adiponectin,C-reactive protein(CRP),tumor necroses factor-a(TNF-a) and fibrinogen(Fbg) were detected and analyzed in 61 patients with uremia by ELISA.Result The plasma CRP levels in 47.54% patients exceeded 5 mg/L(the normal reference value).In the trial group the plasma levels of adiponectin,tumor necrose factor-a and fibrinogen were higher than those in healthy controls(P<0.05).ADPN was negatively correlated with CRP,TNF-a,Fbg and Coefficient of correlation is-0.49,-0.51and-0.43,respectively(P<0.01).Conclusion The plasma adiponectin concentrations are closely correlated with internal inflammatory reaction,anaemia and malnutrition in uremia patients and adiponectin may be a protective factor.
