宫颈AutoCyte PREP检查结果8750例的临床分析

摘    要:目的通过对宫颈超柏氏液基薄层细胞学技术(AutoCyte PREP,LCT)检查结果的分析,探讨宫颈阴道细胞学疾病的临床特点。方法回顾性分析2006年3月-2008年12月间我院开展的8750例宫颈LCT检查结果,细胞学诊断按TBS标准。将LCT细胞学异常结果与阴道镜下组织活检结果相比较。结果8750例LCT检查结果中,发现异常涂片766例,其中,LSIL占1.074%,HSIL0.480%,宫颈鳞癌0.0457%,宫颈腺癌0.00%。ASCUS6.800%,ASCH0.194%及不典型腺上皮细胞(AGus)0.160%。LCT结果为LSIL、HSIL和SCC的140例患者与活检结果的总诊断符合率为78.571%。其中LSIL诊断符合率为75.596%,HSIL诊断符合率为80.952%,SCC诊断符合率为100%,3组间诊断符合率的差异有统计学意义(x^2=10.02,P=0.0076)。结果表明病变程度越高,LCT的TBS细胞病理学与组织病理学的诊断符合率也越一致的现象。结论AutoCytePREP提高了各种宫颈细胞学疾病的检出率及准确性。

Abstract:Objective To evalute the effects of screening the cervical epithelial cell abnormalities by using the liquid-based, thin-layer test(AutoCyte PRIMP System, LCT). Methods To retrospectively study 8750 cases of LCT results from 2006 to 2008. The diagnosis was in accordance with the bethesda system(TBS)standard.Abnomud reports of LCT were compared with that of biopsies under -colposcopy. Results In the 8750 cases, 766 cases of the cervical epithelial cell abnonnalilties were found, in which, there were 1.074% of LSIL,0.480% of HSIL,0.0457% of Squmaous cell carcinoma,0.00% of Glandular cell carcinoma,6.800% of ASCUS,0.194% of ASCH and 0. 160% of AGUS. The total coincidence rates of LCT with biopsies were 78. 571%. The coincidence rates of LCT with biopsies were 75.596% in LSIL,80.952% in HSIL and 100% in cervicalcancer. There is significant difference between the three groups ( Z2 = 10.02, P = 0.0076). Conclusion AutoCyte PREP cytology demonstrated significantly increased sensitivity and accuracy for detecting cervical lesions.
