常见血液病的脾切除治疗

全文介绍几种常见血液病共76例的脾切除治疗经验。认为只要术前适当纠正贫血、出血,改善周围血细胞,注意手术操作尤其是创面渗血,手术安全性较大,全组无1例手术死亡。然血液病病因复杂,疗效悬殊,以自身免疫性溶血性贫血、原发性血小板减少性紫癜较为理想。掌握手术指征尚是难题。副脾部位常有变异,应寻找切除,否则影响疗效。血小板增多宜重视,但不一定都发生血栓。

Splenectomy for Some Hematologic Diseases

76 patients with hematologic disease(including auto immune hemolytic ane-mia, AIHA, 13, idiopathic thrombocytopenic purpura, ITP, 26, chronic myelo-idleukemic CML 11 and aplastic anemia 26 cases)have been treated with splectomy since 1975. No patient died in the early postoperative period. The main complication during operation was subcutaneous bleed. Postoperative wound infection and thrombocytosis may be influenced the result of treatment. According to our experience, splenectomy will give better improvement in patients with AIHA and ITP than the others. Nowadays, it demauds already an appreciation of the hema-tologic disease process by surgeon himselfs in order to give the accurate operative indication.
