慢性乙型肝炎血清LHBs与HBV DNA、HBeAg、ALT的相关性

目的探讨HBV表面大蛋白(LHBs)与HBV DNA、HBeAg、ALT的相关性及LHBs指标在慢性乙型肝炎(CHB)诊治中的临床意义。方法选择324例CHB患者,分别检测LHBs、HBV DNA、HBeAg、ALT。血清LHBs和HBeAg采用ELISA法,HBV DNA检测采用荧光定量PCR法。结果 324例CHB患者血清中,随着HBV DNA载量的升高,LHBs的浓度呈上升趋势,两者之间有良好的相关性(r=0.97,P<0.01),且LHBs的阳性率也逐步增加,不同组别间LHBs阳性率差异均有统计学意义(P<0.05);在171例HBeAg阴性血清中,LHBs和HBV DNA的阳性率分别为66.08%和61.99%(P>0.05);171例HBeAg阴性血清中,113例LHBs阳性与58例LHBs例阴性组血清ALT有统计学差异(P<0.05)。结论 LHBs是反映HBV感染者,特别是HBeAg阴件患者体内病毒复制程度敏感、可靠指标;检测CHB患者血清中LHBs对于了解乙型肝炎肝细胞损伤程度、抗病毒疗效和评估预后具有一定的临床价值。

Correlation of LHBs and HBV DNA, HBeAg, ALT in CHB Patients

Objective To investigate the correlation of hepatitis B virus large surface protein(LHBs) and HBV DNA,HbeAg and ALT and the clinical significance of LHBs indicator in the diagnosis and treatment of chronic hepatitis B virus(CHB).Methods Serum LHBs,HBV DNA,HBeAg,and ALT were detected with ELISA and fluorescence quantitative PCR method in 324 patients with CHB.Results Serum concentration of LHBs in CHB 324 cases had an increasingly trend with the increase of copy number of HBV DNA,which were well correlated(r=0.97,P<0.01).The positive rate of LHBs was increased gradually as well,and the difference of LHBs positive rate among different groups was significant(P<0.05).The positive rates of serum LHBs and HBV DNA were 66.08% and 61.99%,respectively,in 171 cases of HBeAg negative(P>0.05).In 171 cases of HBeAg negative,the difference of serum ALT was significanltly different between LHBs positive(113 cases) and LHBs negative(58 cases)(P<0.05).Conclusion Detecting LHBs may sensitively and reliablly reflect the level of in vivo virus replication of HBV,especially in the patients with HBeAg negative.Examination of serum LHBs in CHB patients has certain clinical value for evaluating the liver damage and anti-virus therapeutic effect,and predicting the prognosis.
