脾动脉超声检测评估慢性病毒性肝炎患者肝纤维化及炎症程度的价值

目的探讨脾动脉超声检测评估慢性病毒性肝炎患者肝纤维化及炎症程度的价值。方法用彩色多普勒超声检测了352例慢性病毒性肝炎伴不同程度肝纤维化患者的脾脏厚度(SPT)、脾动脉收缩期峰值速度(VsSPA)及脾动脉阻力指数(RISPA)、搏动指数(PISPA)、血流量(SPFA);结合肝组织活检病理学诊断并对其结果进行分析。结果超声测得的不同炎症程度分期(G1,G2,G3,G4期)的SPT、VsSPA及SPFA值均随病变分期增高而明显增大(P<0.05),不同肝纤维化程度分期(S0,S1,S2,S3,S4期)的SPT和RISPA均随病变分期增高而明显增大(P<0.05)。结论脾动脉超声检测能可靠评估慢性病毒性肝炎患者肝纤维化及炎症程度。

Significance of ultrasound splenic artery examination in evaluating the severity of hepatic fibrosis and inflammation in patients with chronic viral hepatitis

Objective To investigate the significance of ultrasonic examination of the splenic artery in evaluating the severity of hepatic fibrosis and inflammation in patients with chronic viral hepatitis. Methods The indexes of spleen thickness(SPT),splenic artery peak systolic velocity(VsSPA),resistance(RISPA),pulsatility(PISPA) and blood flow(SPFA) were meassured with color Doppler ultrasound in 352 patients with chronic viral hepatitis.The results were analyzed among different pathological stages of hepatic fibrosis(S0,S1,S2,S3,S4) and inflammation (G1,G2,G3,G4) obtained by hepatic biopsy. Results The values of SPT,VsSPA and SPFAD were enlarged as S stage of hepatic inflammation increased(P<0.05).So did the values of SPT and RISPA as G stage of hepatic fibrosis increased(P<0.05). Conclusion The indexes of ultrasound splenic artery examination are reliable in evaluating the severity of hepatic fibrosis and inflammation in patients with chronic viral hepatitis.
