Evaluation of left atrial active pump function with strain rate imaging in patients with lone paroxysmal atrial fibrillation

Objective To evaluate left atrial（LA） active pump function in patients with lone paroxysmal atrial fibrillation（PAF） and the impact of catheter radiofrequency（RF） ablation with stain-cate imaging.Methods Twenty patients with lone PAF and undergone successful catheter RF ablation（PAF group,age（59 ± 11） years,LVEF（65.65% ± 5.79%） were enrolled.The LA movement and deformation,tissue velocity imaging（TVI） and strain rate imaging（SRI） were obtained before,3 months and 6 months after the ablation procedure.The peak late-diastolic velocity/strain rate,the active atrial displacement/strain pre-P wave and its percentage of the total displacement/strain were measured with the region of interest located in the middle part of interatrial septum（IAS） and LA lateral walls.Twenty normal subjects for control group,age（58 ± 10） years,LVEF 67.60% ± 6.44% were also analyzed.The parameters of LA pump function were compared between the two groups,and were monitored after RF ablation.Results LA were dilated in lone PAF with decreased LA wall peak late-diastolic velocities and the percentage of active displacement/strain revealed from TVI and SRI,suggesting the abnormality of LA movement and deformation function.RF ablation could successfully reserve LA remodeling.However,LA pump function was not improved significantly.Most functional indices remained abnormal even 6 months later.Conclusion SRI provides a simple and feasible tool for quantifying atrial function.Dilated LA with damaged movement and deformation function is revealed in apparent ＂lone＂ PAF.In despite of the rapid reversion of LA remodeling,the abnormality of LA active pump function may last more than 6 months after a successful RF ablation,implicating the necessity of long-term clinical treatment and follow-up.
