腹腔镜下阻断子宫动脉在子宫腺肌瘤切除术中的临床应用研究

目的探讨腹腔镜下阻断子宫动脉并行子宫腺肌瘤切除术的应用价值。方法 83例患者随机分为观察组41例和对照组42例,观察组先行腹腔镜下子宫动脉阻断后行腺肌瘤切除术,对照组直接行腹腔镜下子宫腺肌瘤切除术。观察两组手术时间、术中出血量、住院天数、术后病率、术后复发率,痛经症状缓解情况。结果观察组术中出血量明显少于对照组,手术时间明显长于对照组,差异有统计学意义（P〈0.05）,平均住院日、术后病率两组差异无统计学意义（P〉0.05）。观察组术后痛经缓解率为78.05%（32/41）,对照组术后痛经缓解率为54.76%（23/42）,二者比较差异有统计学意义（P〈0.05）。术后复发率二组间差异有统计学意义（P〈0.05）。结论腹腔镜下阻断子宫动脉并子宫腺肌瘤切除术是较为理想、安全有效的保留子宫的手术方法,具有创伤小、出血量少、住院时间短、痛经缓解率高、复发率低、并发症少等优点。

The clinical application of Laparoscopic uterine artery occlusion in the uterine gland myomectomy

Objective To explore the concurrent laparoscopic uterine artery occlusion uterine gland momentum application value.Methods 83 patients were randomly divided into two groups of 41 cases and the control group 42 cases,the observation group first laparoscopic uterine artery occlusion across gland momentum in the control group under the direct line of laparoscopic momentum uterine gland.Were observed operating time,blood loss,hospital stay,postoperative morbidity,postoperative recurrence rate,relief of symptoms of dysmenorrheal.Results The observation group was significantly less than the control group,blood loss,operative time was longer than the control group,the difference was statistically significant（P0.05）,the average length of stay,postoperative morbidity was no significant difference between the two groups（P0.05）.Dysmenorrheal remission rate after the observation group was 78.05%（32/41）,the control group after remission rate of dysmenorrheal was 54.76%（23/42）,the difference between the two school terms to unify and significance（P0.05）.Postoperative recurrence rate of 2 statistically significant difference between the groups（P0.05）.Conclusion Laparoscopic uterine artery occlusion and momentum uterine gland is an ideal,safe and effective surgical method to retain the uterus,with less trauma,less bleeding,shorter hospitalization,dysmenorrheal to alleviate the high rate of recurrence rate is low,less complications and so on.
