腺多形性腺瘤1例报告

目的回顾腺多型性腺瘤的组织学特点、鉴别诊断要点和临床特点。方法以我院发生的一例28岁男性，腺多形性腺瘤患者为例，回顾临床病理特点并复习文献。结果腺肿瘤是罕见的。国外报道，腺肿瘤占眼眶肿瘤的35％，而多形性腺瘤占腺肿瘤的24％^[1]，多形性腺瘤的男女比例60：40^[3]。平均年龄39岁^[5]。国内报道与国外报道大体一致。形态学上同唾液腺多形性腺瘤。结论回顾性地看腺多形性腺瘤，组织学诊断并不难。术前通过临床症状和影像学检查後，大部分可确诊为腺多形性腺瘤。手术前诊断腺多形性腺瘤并认识到初次手术时要将瘤体连同包膜一次性完整切除是十分重要的^[8.9]。这样可以避免因取活检或不完整切除导致肿瘤包膜破坏引起的复发或恶变。

Report of Lacrimal Polymorphic Adenomas （One Case） and Review of the Literatures

Objectives to review the lacrimal polymorphic adenomas in the histopathological, differential diagnosis and clinical features. Medthods a case of 28-year old men with mixed tumor was taken and the clinical pathological features were analyzed and literatures reviewed. Results Polymorphic adenomas of the lacrimal gland are seen rarely. Overseas reports shows that lacrimal tumors was 35% in orbital tumors while mixed tumor was 24% in lacrimal tumors[l]; it was 60：40 in men and women [3]. Average age was 39 [5]. Reports in China were consistent with it approximately. Morphological features of the lacrimal polymorphic adenomas wereconsistent with saliva glands＇ Conclusion To look back polymorphic adenomas, the histological diagnose is not difficult. According to clinical symptom and radiological features polymorphic adenomas mostly can be diagnosed before surgery. It is very important to completely remove the tumor in its capsule in the initial surgery that can avoid recurrence and malignant change caused by biopsy made before the operation or destruction of the capsule in operation.
