甲状腺功能亢进症病人围手术期血清甲状腺激素水平变化的研究

目的：探讨围手术期碘荆对患者体内甲状腺素的影响。方法：60例患者均经内科门诊口服抗甲状腺药物治疗1个月以上，然后转外科。用放免法测定手术前后病人不同时间的T3、T4、TSH的变化。结果：本组全部病例病情均稳定控制，甲状腺缩小、变硬，如期手术，无危象及呼吸道并发症发生，治愈率100％。T3、T4、TSH服碘前和术晨比较无显著性差异，但术后24h、48h下降明显（P〈0．01）。结论：碘剂的应用是甲亢术前准备用药中最安全可靠的方法，术后服碘已无实际意义。

Changes on The Level of Perioperative Serum Thyroid Hormone in Patients with Hyperthyreosis

Objective:To study the effect of iodides on thyroid hormone in perioperative hyperthyroidisis patients.Methods:60 patients with hypertbyreosis took oral antithyroid drug over l month before performing operation.The changes of T_3 and T_4 and TSH were measured with radioimmunological assay before and after operation.Results:All patients condition was ideally controlled with decreasing and sclerosed thyroid gland.There was no thyroid crisis and respiratory complications occured.The cure rate was 100%.The levels of T_3,T_4 and TSH showed no differences before taking iodides and in the morning of date of surgery,but decreased significantly at 24 hours and 48 hours after operation.Conclusion:The application of iodides is safe and reliable before operation,but it is meaningless after operation.
