机械通气在小儿吸入全麻下行纤维支气管镜检术中的应用

目的：探讨机械通气在小儿吸入全麻下行纤维支气管镜检术中应用的可行性和安全性。方法：60例需行纤维支气管镜检气管内异物取出术患儿,按术中不同通气方式随机分为自主呼吸组（地卡因表麻后保留自主呼吸,吸入七氟烷全麻）和机械通气组（静脉快通道诱导,气管内插管后吸入七氟烷全麻）组,每组30例。监测两组纤维支气管镜操作时长、术毕清醒时长、术中并发症情况及操作满意度。结果：两组均完成纤维支气管镜检操作。与自主呼吸组比,机械通气组纤维支气管镜操作时长显著缩短、操作满意度显著提高（P〈0.05）,术中无并发症,术毕清醒时长两组比无统计学意义。结论：采用气管内插管全麻机械通气方式下行纤维支气管镜检术是一种更为安全有效的麻醉方法。

Application of Mechanical Ventilation in Children Bronchofiberscopy Undergoing Inhalation General Anesthesia

Objective：To explore the feasibility and safety of application of mechanical ventilation in children bronchoscopy undergoing inhalation general anesthesia. Methods：Sixty children undergoing bronchofiberscopy tracheal foreign body removal were divided into spontaneous breathing group（inducted and maintained with sevoflurane after topical anesthesia with tetracaine,n=30）and mechanical ventilation group（inducted by fast channel,inhalated sevoflurane after tracheal intubation,n=30）,according to the different ventilation modes. The operation time,recovery time,intraoperative complications,operation satisfaction were recorded. Results：The operation of bronchofiberscopy of two groups were finished. The shorter operation time and more operation satisfaction were gotten in the mechanical ventilation group（P〈0.05）,compared with spontaneous breathing group,no complication was founded when operating. There was no difference in recovery time between two groups. Conclusion：It is a more safe and effective method in tracheal intubation general anesthesia mechanical ventilation by bronchofiberscopy.
