急诊内镜检查在上消化道出血中的应用

目的探讨急诊内镜检查在上消化道出血患者中的临床应用价值。方法回顾性分析2003年10月至2007年5月因上消化道出血于48h内行急诊内镜检查159例患者的临床资料，对比分析不同时间内镜检查的差异以及检查前冰盐水洗胃与否对检查结果的影响。结果急诊内镜检查确诊率为94．97％，不同时间内镜检查确诊率不同，发生出血后24h内行内镜检查其确诊率明显提高，与24～48h相比差异有显著性（P〈0．01）。内镜检查前洗胃与否两组对比分析无明显差异（P〉0．05），所有患者均未发生严重并发症。结论急诊内镜检查是安全有效的，尽早行内镜检查可提高诊断准确率，内镜检查前无需冰盐水洗胃。

Analysis of the application of emergency endoscopy in 159 cases with upper gastrointestinal bleeding

Objective To evaluate the value of clinical application for emergency endoscopy in upper gastrointestinal bleeding. Methods Retrospectively analyzed 159 cases with upper gastrointestinal bleeding from oct, 2003 to may, 2007, they were received emergency endoscopy within 48 hours in our hospital, investigate the difference by emergency endoscopy examination during different time and using or not gastric lavage prior to emergency endoscopy. Results The correct diagnosis rate by emergency endoscopy was 94. 97% , and it was higher within 24 hours than that in 24-48 hours (P<0.01),there were no difference between using gastric lay-age or not(P>0.05). No severe complications were happened. Conclusion Emergency endoscopy is a safe and useful method to diagnosis upper gastrointestinal bleeding. It is not necessary using gastric lavage prior to e-mergency endoscopy.
