糖皮质激素治疗口腔黏膜扁平苔藓56例的临床体会

目的观察糖皮质激素治疗口腔黏膜扁平苔藓的临床疗效。方法选取在该院口腔科2010年5月-2011年6月收治口腔黏膜扁平苔藓患者56例，随机分为治疗组和对照组，治疗组采用地塞米松注射液进行治疗，对照组采用西瓜霜喷雾剂缓解症状，治疗4周，记录患者的治疗效果，分析血清IL-6、IL-8和TNF-α水平。结果两组患者在一般资料中无统计学差异（P〉0．05）；治疗组总有效率92．9％，对照总有效率60．7％。治疗组明显高于对照组，两组比较差异有显著性（P〈0．01）。对照组和治疗治疗前后TNF-仪、IL-6和IL-8均较治疗前有差异性（P〈0．05），治疗后治疗组TNF-α和IL-6均优于对照组（P〈0．05），IL-8指标显著优于对照组（P〈0．01）。结论糖皮质激素能快速有效地改善口腔扁平苔藓患者免疫功能，值得临床广泛应用。

Clinical efficacy of glucocorticoid homeone therapy on 56 cases of oral mucosal lichen planus

Objective To observe the clinical efficacy of glucocorticoid hormone therapy on oral mucosal lichen planus. Methods From May 2010 to June 2011,56 oral mucosal lichen planus patients were treated in our hospital,who were randomly divided into experimental group and control group. The experimental group received dexamethasone injection , and control group received watermelon frost spray to re lieve symptoms. Treatment lasted for 4 weeks, patient outcomes were recorded, and analysis was made of IL-6, IL-8 and TNF-alpha levels in serum. Results Between two groups of patients there was no significant difference （P 〉 0.05）. In the experimental group,the total effec tive rate was 92.9%, which was significantly higher than that in the control group, 60.7% （P 〈 0.01 ）. The control group and the experi mental group had significant differences before and after treatment （P 〈 0.05）. TNF-alpha and IL-6,in the treatment group were better than those in the control group （P 〈 0.05）, IL-8 was significantly better than that in the control group （P 〈 0.01 ）. Conclusions Glucocorti coids could quickly and effectively improve the immune function of patients with oral lichen planus,which was worthy of wider application.
