长期吸入糖皮质激素对支气管哮喘患儿可溶性细胞间黏附分子-1的影响

摘    要:目的探讨支气管哮喘患儿长期吸入糖皮质激素后对可溶性细胞间黏附分子-1(sICAM-1)水平的影响。方法采用酶联免疫吸附(ELISA)法检测36例健康儿童(正常对照组)和29例支气管哮喘患儿吸入糖皮质激素(布地奈德气雾剂)治疗前及后(3、6、12个月)血清sICAM-1水平变化。结果1.支气管哮喘患儿治疗前血清sICAM-1水平明显高于对照组,有极显著性差异(q=34.12 P<0.01);2.吸入糖皮质激素后3、6和12个月血清sICAM-1水平逐渐降低,各治疗组间有极显著性差异(P均<0.01),治疗后各组与治疗前比较均有极显著差异(P均<0.01),其中治疗后3、6个月患儿血清sICAM-1水平明显高于对照组(P均<0.01),而12个月与正常对照组比较无显著差异。结论黏附分子在支气管哮喘的发病中起重要作用。糖皮质激素可能通过抑制sICAM-1水平升高而起到缓解及预防哮喘发作的作用。

Abstract:Objective To explore influence of long-term inhaled glucocoticoids(IGS) on soluble intercellular adhesion molecule-1(sICAM-1)) in children with bronchial asthma.Methods Enzyme-linked immunosorbent assay(ELISA) method was used to detect the serum sICAM-1 level in 36 healthy children and 29 children with bronchial asthma(untreated and post-treated for 3,6 and 12 months).Results 1.Serum sICAM-1 level was significantly elevated in children with asthma and significantly higher than that in normal control group(P<0.01).2.The level of sICAM-1 in children with bronchial asthma decreased gradually after inhaled glucocoticoids 3,6 and 12 months,and there was significant different compared with that in untreated group(P<0.01),the levels of sICAM-1 after IGS 3 and 6 months were obviously different compared with those of normal control group(P<0.01).But there was no difference of the serum sICAM-1 level in 12 months compared with that of normal control group.Conclusions The adhesion molecules play an important role in asthma.IGS can inhibit and prevent asthma by inhibiting the serum sICAM-1 level.
