胰腺导管内乳头状瘤的CT与MRI诊断

目的:探讨胰腺导管内乳头状肿瘤的CT和MRI的影像学特点,旨在提高该病的影像诊断符合率。方法:回顾性分析经手术病理证实的8例胰管内乳头状黏液性肿瘤的CT和MRI表现,其中男6例,女2例,平均年龄61.3岁。术前8例均行CT平扫及增强检查,2例行MR检查。结果:CT检查发现胰头部囊实性混合肿块5例,胰体尾部混合密度肿块3例,MRI检查发现胰头部囊实性混合肿块2例;肿块最大径2.1cm~4.2cm不等,增强后表现为不均匀强化。6例均有不同程度的胰管扩张,十二指肠乳头增大2例。结论:胰腺导管内乳头状黏液性肿瘤在CT和MRI上有一定的特点,有助于该病的诊断和治疗。

CT and MRI Features of Intraductal Papillary Mucinous Tumors

Objective To evaluate the CT and MRI features of intraductal papillary mucinous tumors of the pancreas(IPMT),in order to improve the diagnostic accuracy.Methods The CT and MRI appearances were analyzed retrospectively in 8 patients with pathologically proved IPMT.There were 6 men and 2 women,with the mean age of 61.3 years.CT was performed in all 8 cases,MRI was undertaken in 2.Results CT demonstrated the cystic and solid lesions in the head of pancreas in 5 cases and 3 masses in the others of pancreas,MRI demonstrated the cystic and solid lesions in the head of pancreas in 2 cases.The diameter arranged from 2.1 cm to 4.2 cm.Inhomogeneous enhancementwas revealed in contrasted CT and MRI.Different extent dilatation of the main pancreatic duct was demonstrated in 6 cases.Abnormal protruding papilla of Vater was seen in 2 of the 8 cases.Conclusion Certain CT and MRI features may suggest the diagnosis of IPMT of the pancreas,and is helpful to treatment planning.
