消化性溃疡2331例胃镜分析

目的回顾性探讨3个不同时期的消化性溃疡胃镜下的特征.方法自1985年1月至2008年12月,玉溪矿业医院内二科进行了6 351例胃镜检查,检出消化性溃疡2 331例,检出率36.70%.2 331例消化性溃疡的内镜检查,按受检年份分为1985年至1989年、1990年至1999年、2000年至2008年3个时期进行统计分析.结果1985年至1989年、1990年至1999年、2000年至2008年消化性溃疡胃镜检出率分别为45.29%、40.60%、25.93%,组间差异有统计学意义（P〈0.01）,消化性溃疡病并出血的比例（胃镜检出率）分别为15.57%、14.53%、4.19%.消化性溃疡并癌变的比例（胃镜检出率）分别为2.07%、3.00%、4.19%（P〉0.05）.胃溃疡好发部位为胃窦,十二指肠溃疡好发部位为球部.结论消化性溃疡及溃疡并出血胃镜检出率下降,消化性溃疡癌变胃镜检出率有升高趋势.

Analysis of Gastroscopic Examinations in 2331 Cases of Peptic Ulcer

Objective To investigate retrospectively the gastroscopic characteristics of peptic ulcer in three different periods：1985～1989,1990～1999,and 2000～2008.Methods From January 1985 to December 2008,6351 gastroscopic examinations were performed in our hospital,from which 2 331 cases of peptic ulcer were detected.The detection rate was 36.70%.The gastroscopic findings were then analyzed statistically according to the three periods of the examination： 1985 ～1989,1990 ～1999,and 2000 ～2008.Results The detection rates of peptic ulcer under gastroscopy during periods 1985～1989,1990～1999,and 2000～ 2008 was 45.29%,40.6%,and 25.93% respectively（P 0.01）.The proportions of the peptic ulcers complicated with bleeding were 15.57%,14.53%,and 4.19% respectively,and those of peptic ulcers complicated with carcinomatous change were,2.07%,3.00%,and 4.19% respectively（P 0.05）.The predilection site for gastric ulcers was the gastric antrum,and that for duodenal ulcers was the bulb.Conclusion The detection rates of peptic ulcer and peptic ulcer with bleeding are decreasing through the years,while that of peptic ulcer complicated with carcinomatous change shows an increasing trend.
