高血压并脑梗死患者胰岛素抵抗及血脂异常的研究

目的研究高血压并脑梗死患者胰岛素抵抗(IR)、血脂异常的状况及IR与脑大小血管病变的关系。方法测定85例高血压并脑梗死患者(腔隙性脑梗死组40例,血栓形成性脑梗死组45例)的血脂、空腹血糖、空腹血胰岛素及胰岛素敏感指数(ISI),并与40名健康对照者进行比较。结果高血压并脑梗死患者血脂、血糖、胰岛素I、SI与对照组比较均有显著性差异(P<0.01或P<0.05),腔隙性脑梗死组胰岛素明显高于血栓形成性脑梗死组(P<0.05),而ISI较血栓形成性脑梗死组明显减低(P<0.05)。结论高血压并脑梗死存在IR,IR可能是其独立危险因素之一,腔隙性脑梗死可能与IR关系更为密切。

INSULIN RESISTANCE AND LIPPID ABNOMALITY IN PATIENTS WITH HYPERTENSION AND CEREBRAL INFARCTION

Objective To study the insulin resistance(IR) and lipid abnormality in patients with hypertension and cerebral infarction and the relationship between IR and cerebrovascular lesion.Methods The blood lipid,fasting blood-glucose,fasting blood insulin and insulin resistive index(IRI) in 85 cases of patients with hypertension and cerebral infarction(including 40 lacunar cerebral infarction and 45 cases of thrombotic cerebral infarction) in the treatment group were examined,and the results were compared with those in 40 healthy cases in the contrast group.Results Significant differences existed between the two groups in terms of blood lipid,blood-glucose,insulin and ISI(P<0.01 or 0.05).The content of insulin was much higher in the lacunar infarction group than in the thrombotic group(P<0.05),while ISI was much lower(P<0.05).Conclusion IR may be an independent risk factor in patients with hypertension and cerebral infarction.Lacunar infarction may be closely related to IR.
