肝移植术后细菌感染的常见病原菌及其耐药性

摘    要:目的 探讨肝移植术后细菌感染的常见病原菌及其耐药性 ,为肝移植术后细菌感染的经验性防治提供用药依据。方法 对 2 4例肝移植、肝肾联合移植患者 1个月内胆汁、引流液、咽拭子等标本培养阳性的细菌种类、耐药性及其发生时间进行分析。结果 肝移植术后细菌感染以金黄色葡萄球菌、阴沟杆菌、溶血性葡萄球菌、屎肠球菌最常见 ,对临床常用的抗生素耐药性高 ,革兰阳性菌对万古霉素敏感性高 ,革兰阴性菌对替考拉宁和泰能敏感性高。结论 肝移植早期积极的细菌培养对临床抗生素的应用有指导意义。肝移植术后细菌感染以金黄色葡萄球菌、阴沟杆菌、溶血性葡萄球菌、屎肠球菌最常见 ,临床可疑细菌感染而尚未获得培养结果可经验性加用泰能或万古霉素、替考拉宁。

Abstract:Objective: To investigate the distribution of bacterial pathogen and their antibiotic resistance in the patients with bacterial bacterial infections after liver transplantation. Methods: The distribution and bacterial resistance profile of 412 strains of bacterial isolates from 24 inpatients with bacterial infection after liver transplantation. Results: Among 412 strains, S. aureus, E. cloacae, S. haemolyticus, Enterococci faecals were the common pathogens, Their antibiotic resistance was high. The resistant rates Gram positive bacteria to Tiecoplanin or Vancomycin were low and the resistant rates of Gram negative bacteria to Imipenem were low as well. Conclusions: It is important to bacterial surveillance on bacterial resistance early after liver transplantation. S. aureus, E. cloacae, S. heamolyticus, Enterococc faecals were the common pathogens in the patients with bacterial infections after liver transplantation. When bacterial infection was suspected and the results of bacterial culture were not attained, Imipenem or Vancomycin/Tiecoplanim may be used.
