氩氦刀冷冻治疗兔VX2肝肿瘤的疗效分析

 目的 探讨氩氦刀治疗兔VX2肝脏肿瘤的疗效。方法 兔VX2肝肿瘤模型兔30只随机分为A、B和C组，每组10只，分别给予假手术、手术切除和氩氦冷冻，干预后第14、28天行CT平扫和增强检查，观察3组CT动态表现，并比较3组肝内转移有无差异。结果 术后14天C组表现为消融区低密度、无强化，消融区边缘呈环形强化。术后28天，两组消融区平扫均表现为低密度区；强化扫描时，部分边缘未见强化，部分边缘强化环较前扩大，病理提示强化部分有残瘤形成。治疗后35天，病理证实B、C两组肿瘤原位完全清除率比较差异无统计学意义(P>0.05)； A、C两组肝内转移率差异有统计学意义(P<0.05)。结论 氩氦刀治疗能够杀灭肿瘤细胞，不会增加肿瘤的转移概率，CT可作为评价肝癌冷冻治疗疗效的有效手段。

Ar-He (argon-helium) Cryoablation Treatment for Implanted VX2 liver Tumor in Rabbit Model

Objective To investigate the therapeutic effect of Ar-He cryoablation treatment on implanted VX2 liver tumor in rabbit model.Methods Thirty rabbits with VX2 liver carcinoma were divided into group A，B and C randomly (each group n=10)，they were treated by sham operation，hepatic resection or cryotherapy 14 days after implantation，respectively. Nonenhanced and dynamic contrast material enhanced computed tomography(CT) in 14 and 28 days after treatment.The CT manifestations of VX2 carcinoma in three groups were observed and the metastatic rate of three groups within liver was compared.Results In the 14 days after cryoablation，all cryolesions exhibited low density，no enhancement，accompanied by ring enhancement in margin.In the 28 days after cryoablation，plain scan showed low density in cryolesions in group C，in which some lesionsmargins without ring enhancement but others revealed ring enhancement with extension，which pathologically demonstrated residual tumor.The liver metastatic rate in C group was significantly lower compared with that in A group (P<0.05) in the 35 days after treatment.Conclusion Pathology demonstrated that VX2 carcinoma was completed destroyed and liver metastasis was reduced after Argon Helium cryoablation.CT is valuable in assessing the therapeutic effect of Argon-Helium cryoablation for liver cancer.
