脑血管病合并感染临床分析

摘    要:目的 探讨感染对脑血管病发生发展的影响。方法 回顾分析 115 6例脑血管病感染住院患者临床资料 ,并对脑出血、脑梗死起病前后感染情况进行比较。结果 ①脑出血组合并感染患者 ( 2 0 6 2 % )显著高于脑梗死 ( 9 34% ) (P <0 0 1) ,两者都以起病后合并感染居多。②感染以细菌为主 ,呼吸道感染占首位( 5 2 5 % ) ,泌尿道感染其次。③ 115 6例患者院内死亡 76例 ,病死率 6 5 7% ,感染组较非感染组病死率高 (P<0 0 1)。④脑血管病有感染者比无感染者血白细胞计数明显增多 (P <0 0 1)。⑤脑出血与脑梗死的部位与感染的发生有相关性 (P <0 0 5 )。结论 脑出血患者感染的发生率高于脑梗死 ,脑血管病患者起病后合并感染是其致残、致死的重要原因之一。

Abstract:Objective To investigate the infuence induced by infection in the onset and course of cerebrovascular disease in a retrospective cohort study of patients.Methods The data about clinical materials was investigated in 1156 patients with cerebrovascular disease,admitted to our hospital.So as to analyze the relationship between infection and cerebrovascular disease.The case control study between cerebral hemorrhage(CH) and cerebral infarction(CI) was compared.Results (1)Infections were more common in CH(20.62%) than in CI patients(9.34%)(P<0.01).Infections were more frequent after ictus of stroke in CI and CH patients.(2)The infections were mostly origined from bacterial,respiratory system infections were the most common in 52.5% of those patients,secondly,urinary infections.(3)The inhospital mortality of those patients was 6.57%(76/1156).Mortality was higher among patients with infections than patients without infections.(4)Leukocyte counts were markedly higher in the cerebrovascular disease patients with infections than those without infections(P<0.01).(5)There was relationship between the regions of CH or CI and infections.Conclusion The infection rate in the CH group was higher than that in the CI group,the infections after onset of the stroke may be one of the cause of increased disablement and mortality.
