益生菌肠内营养制剂在胃癌术后患者应用的临床研究

目的:探讨及评价微生态营养制剂在临床应用的效果及前景。方法:60例胃肿瘤患者,随机分为四组,术前1d检测血清转铁蛋白行营养状态评估,术后分别给予常规补液(A)、肠外营养(B)、普通肠内营养(C)及添加微生态制剂的肠内营养(D)。监测各组患者手术前1d及手术后7d外周血内毒素、肿瘤坏死因子(TNF)水平变化及淋巴细胞计数改变,同时以细菌学方法检测各组患者手术前后粪便菌群失调情况并分类。结果:术后C组及D组患者均低于A组和B组(P<0.01),而D组内毒素水平低于C组(P<0.01)。各组患者术后肿瘤坏死因子水平均低于术前水平,且D组肿瘤坏死因子水平又明显低于其他各组(P<0.01),A、B、C三组手术前后外周血淋巴细胞计数差异均无显著性(P>0.05),D组患者术后外周血淋巴细胞计数较术前增加(P<0.05)。各组患者术后第一次排便后细菌油镜计数结果C组及D组菌群失调患者例数明显少于A组及B组(P<0.01),而D组又低于C组(P<0.05)。结论:微生态肠内营养可以增强胃恶性肿瘤术后患者的机体免疫及降低感染概率。

Clinical Study of Enteral Microecopharmaceutics Applying on Post-operative Patients Suffered Stomach Tumor

Objective To approach the clinical effects of enteral microecopharmaceutics on post-operative patients suffered stomach tumor.Methods 60 cases with stomach tumor were separated randomly into four groups,normal liquid group(A),parenteral nutrition group(B),enteral nutrition group(C)and enteral microecophar-maceutics group(D),which received different post-operative nutrition treatment.The perioperative changes of endotoxin,TNF and the number of lymphocyte in peripheral blood were analyzed,and the conditions of dysbacteriosis were detected.Results The lever of endotoxin in serum markedly decreased in the C and D group compared with A and B group,and in the D group,the lever of endotoxin is lower than the C group;The average of TNF in serum is decreased in each group after operation,in addition,the D group was lower than the other groups;The number of lymphocyte in serum increased after operation in the D group,the other groups have not significant alternation;The patients with intestinal microflora disorders in C and D group less than in A and B group,in addition,the D group was less than the C group.Conclusion Enteral microecopharmaceutics can improve the post-operation immunoreaction and decrease the post-operation infection rate of patients with stomach Tumor.
