腹膜透析清除慢性肾衰竭患者血清瘦素及炎症因子效果研究

目的观察持续性不卧床腹膜透析(CAPD)治疗尿毒症患者高瘦素血症及清除炎症因子效果,旨在改善CRF患者的生存质量。方法 30例CAPD的尿毒症患者为治疗组,正常健康体检者30例做对照组。观察尿毒症患者治疗前、透析后1 d、30 d、90 d血清瘦素、TNF、IL-6、IL-8、CRP水平变化。同时观察患者体重指数(BMI)、血压、食欲、精神状态、贫血改善情况,与正常对照组及自身治疗前后对照。结果血清瘦素、TNF、IL-6、IL-8、CRP浓度透前治疗组与正常对照组比较显著增高(P<0.05或<0.01);治疗组与自身透前比较透后1 d、透后30 d、透后90 d显著降低(P<0.01)。治疗组BMI、血压、食欲、精神状态、贫血均有不同程度改善(P<0.05或<0.01)。结论 CAPD清除尿毒症患者血清瘦素及炎症因子作用明显,能改善患者的微炎症状态和营养不良状态,可提高生存质量

The effect observation of peritoneal dialysis on clearance of Hyperleptinemia and inflammaory factor in uremic patients

Objective To observe the effect of continuous ambulatory peritoneal dialysis(CAPD) on treating hyperleptinemia and the clearance of inflammatory factor in uraemic patients in order to improve the quality of life for the patients with chronic renal failure(CRF).Methods 30 uraemic patients treat with CAPD as the treatment group,and 30 healthy ones as the control group.To observed the changing of serum leptin,TNF,IL-6,IL-8 and CRP of uraemic patients before treatment,in one day's,30 days' and 90 days' treatment.And observe the body mass index(BMI),blood pressure,appetite,mental status and the improvement of anemia at the same time.Compared all datas that before and after treatment in the treatment group and compared with that`s of the control group.Results Before the treatment with CAPD,the serum leptin,TNF,IL-6 and IL-8 in the treatment group were significantly higher than the control group,(P<0.05 or 0.01),and they were decreased significantly after the treatment at one day,30 days and 90 days timepoint in the treatment group(P<0.01).The BMI,blood pressure,appetite and mental status in the treatment group were increased in different level(P<0.05 or 0.01).Conclusion There is a significant effect of CAPD on clearance of serum leptin and inflammatory factors,which could improve the light inflammatory reaction,malnutrition and the quality of life for the patients.
