前后联合手术入路治疗累及双柱的髋臼骨折

目的总结应用髂腹股沟（I—I）入路联合Koche-Langenbeck（K—L）入路（前后联合入路）治疗累及双柱严重移位的髋臼骨折的经验。方法对37例应用不同手术入路治疗的累及双柱严重移位的髋臼骨折进行回顾性研究。术后随访6个月至5年，平均3．6年。结果骨折复位情况按Matta评定标准，骨折复位优良率：前后联合入路组80．9％（17／21），单一入路（K—L或I—I）组68．7％（11／16），两组相比差异有统计学意义（P〈0．05）。关节功能按照改良的Merled Aubigne和Postel评分系统进行评估，关节功能优良率：前后联合入路组85．7％（18／21），单一入路（K—L或I—I）组75．0％（12／16），两组相比差异有统计学意义（P〈0．05）。联合入路组21例中严重异位骨化2例，无1例感染。结论前后联合入路治疗累及双柱严重移位的髋臼骨折显露充分、复位方便、固定稳定、临床效果好、并发症少。
