交叉韧带损伤磁共振诊断及临床价值探讨

目的 :探讨膝关节交叉韧带损伤的磁共振表现及其在临床中应用价值。方法 :82例磁共振诊断为交叉韧带损伤进行手术治疗患者 ,急性损伤 6 5例 ,陈旧性损伤 17例。以手术和关节镜为诊断标准 ,本组患者前交叉韧带 (ACL)断裂 2 3例 ,重度撕裂 19例 ,ACL胫骨附着点撕脱骨折 30例 ,后交叉韧带 (PCL)体部断裂 3例 ,PCL胫骨附着点撕脱骨折 7例。结果患者均顺利进行磁共振检查 ,本组ACL损伤的磁共振表现为 :韧带完全断裂 ,断端分离移位 15例。ACL撕裂 2 7例 ,表现为韧带局限性变细 ,撕裂达韧带宽径一半以上 3例 ,撕裂 <韧带宽径一半 19例 ,ACL前倾角增大 ,胫骨轻度前移仅 5例ACL胫骨附着点撕脱骨折 30例 ,表现为ACL胫骨附着处骨块或骨片撕脱伴移位 ,8例骨折畸形愈合表现胫骨关节面隆起。本组PCL损伤的磁共振表现为 :PCL断裂伴断端分离移位 3例 ,PCL韧带附着点撕脱骨折 7例。经与手术结果对照分析 ,ACL损伤诊断正确率为 88% ,撕裂、断裂、撕脱骨折诊断正确率分别为 86 .3%、6 5 .2 %、10 0 %。PCL诊断正确率达 10 0 %。本组 4 5例患者施行了交叉韧带重建术 ,2 9例患者施行了骨折复位内固定术 ,8例施行了骨折畸形愈合复位内固定术。结论 :膝关节磁共振检查对交叉韧带损伤程度、类型诊断较为准确 ,对临床治疗指导价值

MRI Diagnosis for Damaged Cruciate Ligament and Its Clinical Enaluation

Objective: To investigate the manifestations of MRI for damaged cruciate ligament of knee joint and its clinical value?Methods: There are 82 patients with damaged cruciate ligaments of knee joints through MRI diagnosis,including 39 patients with broken cruciate ligaments or dislocation of appendiculate points,21 with Anterior cruciate ligaments broken,18 of back cruciate ligaments broken and 9 with cruciate ligaments broken before?Results: The accuracy of diagnosis for Anterior cruciate ligaments broken was 97%,the accuracy for poster cruciate ligaments broken was 100%. The accuracy of MRI diagnosis was significantly higher than that of physical examination. There were many features of MRI for diagnosing damaged cruciate ligaments including the direct sign of increased signal of the ligaments,the broken continuity of ligament and the location of appendicular joints and the indirect sign of tibia displacing forward. Conclusion:MRI examination for damaged cruciate ligament of knee joint is not only imprrtant for diagnosis but also will guide the treatment?
