轻密度与重密度布比卡因在下肢手术麻醉中的对比分析

目的通过临床对下肢手术患者给予轻密度与重密度的布比卡因麻醉,分析不同计量的布比卡因麻醉效果以及临床使用安全性。方法选取某院2010年1月～2011年1月所收治的下肢手术患者128例随机分为轻密度组和重密度组,每组64人。轻密度组患者麻醉药液配置方法为4ml注射用水+2ml的0.75%布比卡因;中密度组患者麻醉药液配置方法为1ml的5%的葡萄糖溶液+2ml的0.75%布比卡因,观察两组患者的麻醉效果、阻滞时间以及不良症状发生情况。结果两组患者均顺利完成手术,轻密度组患者临床麻醉效果显著的有56例(87.5),麻醉效果良好的有8例(12.5%);重密度患者临床麻醉效果显著的有42例(65.6%),麻醉效果良好的有22例(34.4%),两组比较差异有统计学意义(P﹤0.05)。轻密度组患者的麻醉维持时间和组织平面明显比重密度患者减少(P﹤0.01)。结论临床对下肢手术患者给予轻密度和重密度的布比卡因麻醉具有非常好的镇痛效果,但是轻密度布比卡因在临床应用过程中并不需要改变患者的体位,并且能够稳定患者的呼吸以及循环系统,术后不良反映少,使用安全可靠,值得临床推广与应用。

Comparative analysis on light density and weight density of bupivacaine anesthesia in the lower limbs

OBJECTIVE In lower extremity surgery with the density and weight density of bupivacaine anesthesia,to analyse the different measurement and of bupivacaine anesthesia and its safety in clinical use.METHODS 128 patients with lower limb surgery were treated in a hospital from January 2010-January 2011 and were randomly divided into groups of light and weight density groups.Light-density of anesthesia liquid configuration was 4ml water for injection +2 ml of 0.75% bupivacaine;middle density configuration was 1ml of liquid anesthetic 5% glucose solution +2 ml of 0.75% bupivacaine.The narcotic effects,block time and occurrence of adverse symptoms were observed.RESULTS All patients were successfully operated,56 cases(87.5%)were with remarkable anesthesia effect in light-density group,the anesthetic effect good in 8 patients(12.5%);42 cases(65.6%)were with remarkable anesthesia in weight density group,good anesthesia effect in 22 patients(34.4%),the difference in the two groups was significant(P﹤0.05).The duration of anesthesia and the proportion of tissue surface of patients in light density group were reduced compared with weight density group(P﹤0.01).CONCLUSION Light density and weight density of bupivacaine anesthesia for the patients with lower limb surgery have a very good analgesic effect,but the clinical application of the light density of bupivacaine does not need to change the patient’s position,and be able to stabilize the patient’s respiratory and circulatory system,with less postoperative adverse reactions.It is safe,reliable and worthy of clinical application.
