腹腔镜下胃十二指肠溃疡穿孔修补治疗临床研究

目的比较腹腔镜下胃十二指肠溃疡穿孔修补术和开腹修补术治疗胃十二指肠溃疡穿孔临床效果。方法针对某院2007年3月～2010年6月腹腔镜下和开腹修补治疗胃十二指肠溃疡穿孔患者各48例进行回顾性研究,以开腹手术治疗患者为对照组,腹腔镜手术治疗患者为实验组,比较两组患者术后并发症发生情况,手术时间、术中出血量、术后排气时间、住院时间、下床活动时间、镇痛药使用等围手术期情况及术后溃疡愈合情况。结果实验组患者术后并发症发生率,手术时间,术中出血量,术后排气时间,住院时间,下床活动时间及镇痛药使用率均明显少于对照组,差异有统计学意义(P﹤0.05)。结论与传统开腹手术相比,腹腔镜下修补治疗胃十二指肠溃疡穿孔手术具有快速,微创,恢复时间短,术后并发症少等优点,具有临床推广使用价值。

The clinical research of repair in gastro-duodenal ulcer perforation under celioscope

OBJECTIVE To evaluate the effect of repair in gastro-duodenal ulcer perforation under celioscope.METHODS We retrospectively analyzed 48 cases who had gastro-duodenal ulcer perforation treated under celioscope from March 2007 to June 2010.The cases were given laparotomy in experimental group,other cases were treated under celioscope in control group.Contrasted the complication,surgery time,blood in surgery,exhaust time,time in hospital,time out-of-bed,usage of pain-killer drug and ulcer heal after surgery.RESULTS There were significant differences in the surgery time,blood in surgery,exhaust time,time in hospital,time out-of-bed and usage of pain-killer drug in the both groups(P﹤0.05).CONCLUSION Comparing the laparotomy,repair surgery in gastro-duodenal ulcer perforation under celioscope is rapid,minimally invasive,reduce the time of cure and complication.
