急诊时间对突发急性心肌梗死预后效果的影响

目的探讨突发急性心肌梗死的急诊时间对治疗方法与预后效果的影响。方法根据入院顺序将60名左室突发急性心肌梗死患者随机分为治疗组与对照组(n=30),对治疗组患者在急性突发急性心肌梗死出现症状出现12h内实施介入治疗,对对照组患者在突发急性心肌梗死症状出现12h后实施介入治疗。结果治疗后,两组的EDV、ESV、LVEF都有明显改善,差异有统计学意义(P﹤0.05),但是治疗组的改善程度优于对照组,差异有统计学意义(P﹤0.05)。经过观察与3个月随访,治疗组胸痛与再梗死的发生率明显少于对照组(P﹤0.05)。另外,治疗组没有出现死亡病例,对照组有3例死亡,差异也有统计学意义(P﹤0.05)。结论早期急诊介入治疗突发急性心肌梗死能有效改善超声心动图指标,同时对预后结果有积极影响,值得推广应用。

The time of initial intervention on the prognosis of patients with acute myocardial infarction

OBJECTIVE To investigate the effect of the time of the initial intervention on the prognosis of patients with acute myocardial infarction.METHODS Sixty patients with acute left ventricular myocardial infarction were randomly allocated into two groups(n = 30)based on their order of admission.Patients in the treatment group and the control group were treated by PCI,respectively,within and after 12 h following the onset of the infarction.RESULTS Although EDV,ESV and LVEF had all improved significantly after PCI treatment in both groups(P﹤0.05),and the improvement was more pronounced for patients in the treatment group than those in the control group(P﹤0.05).After 3 months of follow-up,the proportion of patients with chest pain and recurrent infarction was lower in the treatment group than in the control group(P﹤0.05),while no death occurred in the treatment group,three patients died in the control group(P﹤0.05).CONCLUSION Earlier emergency intervention of acute myocardial infarction can effectively improve the echocardiographic indicators and have a positive impact on the prognosis of patients;and its clinical application is highly recommended.
