重建股骨偏心距对人工全髋关节置换术后关节功能影响的临床研究

目的:探讨人工关节置换术中股骨偏心距的重建对关节功能影响。方法:2003年7月~2009年7月,对87例患者行单侧人工全髋关节置换术的患者进行随访,随访时拍X线片测量,对比患侧与健侧股骨偏心距。分析术后偏心距与髋关节Harris评分的相关性,进行χ2检验。结果:81例获得随访,随访时间3~8年,平均5.6年。股骨偏心距恢复(A组)49例,偏心距未恢复(B组)32例。Harris评分优良率A组96.0%,B组为71.8%。偏心距绝对值大小与Harris评分高低无相关关系(p=0.12),偏心距的是否重建与Harris评分优良率方面差异均有统计学意义(p<0.01)。结论:重建股骨偏心距对人工全髋关节置换术后关节功能的恢复有直接影响。

Clinical Study Between Femoral Offset Restoration and Joint Function After Total Hip Replacement

Objective:To study the effect of femoral offset on joint function after total hip replacement.Methods: According to the inclusion criteria,87 patients undergoing unilateral total hip arthroplasty were followed up between July 2003 and July 2009.We took X-ray pictures to compare the femoral offsets at the affected and unaffected hips.The operated hips were evaluated by the Harris hip score.Statistical analyses were performed with SPSS 11.0 software. Results:81 patients were followed up.The average follow up period was 5.6 years. There was not positively relation between the Harris hip score of the operated hip and the absolute value of femoral offset (p=0.12).The difference had significant between whether or not in the rate of femoral offset reconstruction and the Harris hip score. Conclusion:The femoral offset reconstruction is crucial to improve hip function.
