多层螺旋CT肺密度测定与阻塞性肺疾病患者肺功能的相关性分析

摘    要:目的:用多层螺旋CT扫描观察肺密度与慢性阻塞性肺疾病(COPD)患者肺功能的关系。方法:选取2011年10月~2012年2月在我院呼吸科就诊的慢性阻塞性肺疾病患者87例,设为观察组,同时选择同时期的非肺部疾病的患者85例为对照组。所有患者均行多层螺旋CT检查(64层)与肺功能检查,对比肺密度值与FEV1和FEV1/FVC值的关系以及和COPD严重程度分级的关系。结果:比较2组患者各项肺密度值指标,结果观察组平均ALDin值为(-845±42)、平均ALDex值为(-789±53)、平均ALDin-ALDex值为(-56±24)、平均ALDex/ALDin值为(0.92±0.03)、平均(ALDin-ALDex)/ALDin的值为(-0.06±0.02),与对照组比较,差异显著;慢阻肺患者ALDin—ALDex、ALDex、(ALDin—ALDex)/ALDin、ALDex/ALDin等肺密度值与各项肺功能值有显著的差异性。以上均P<0.05。结论:多层螺旋CT能够准确并且便捷的得到肺功能的评价指标,与PFtr检查相关性较好,肺密度值能够筛查及预测COPD患者,临床应用的价值较高。

Abstract:Objective:To observe the relation for lung density by multislice spiral CT scanning and pulmonary function in patients with chronic obstructive pulmonary disease (COPD). Methods: 87 cases of patients with chronic obstructive pulmonary disease for treatment in the department of respiratory of our hospital from October 2011 to February 2012 were chosen as the research object. Set them as the observation group. 85 cases the same period of non-pulmonary disease were chosen as the control group All patients were underwent with multislice CT (64 layers) and pulmonary function tests, the relationship of lung density values with FEV1 and FEV1/FVC values and COPD severity classification were compared. Results: The various lung density value indicators of the two groups were compared. The result of the average ALDin value of the observation group was (-845 ± 42), the average ALDex value was (-789 ± 53), average value was (ALDin-ALDex, -56 ± 24), average ALDex / ALDin value was (0.92 ± 0.03), and average (the the ALDin-ALDex) / ALDin value was (-0.06 ± 0.02). Compared with the control group, there was significant difference. The ALDin-ALDex, ALDex, (lung density values of / ALDin, ALDex / ALDin ALDin-ALDex) of COPD patients and the pulmonary function values had significant differences, and P <0.05.Conclusions: MSCT can get the lung function evaluation in accurate and convenient way, and with a good correlation compared with PFtr examination. The lung density can be screen and predict for the patients with COPD, the clinical application should have higher value.
