摘    要:目的探讨2型糖尿病合并泌尿系感染病原菌的构成及耐药情况。方法采用安徽恒星HX—21细菌鉴定药敏分析仪对某院2007年1月~2010年3月218例门诊及住院2型糖尿病患者合并泌尿系感染病原菌进行鉴定及药敏分析。结果 218株病原菌中,以大肠埃希菌为主(50.46%),其次是肺炎克雷伯菌(11.47%)、铜绿假单胞菌(9.17%)、金黄色葡萄球菌(8.26%)、粪肠球菌(7.34%)、真菌(5.5%)、变形杆菌(4.13%)、凝固酶阴性葡萄球菌(3.67%)。革兰阴性杆菌对头孢哌酮/舒巴坦、亚胺培南及哌拉西林/他唑巴坦耐药率较低,万古霉素对革兰阳性球菌具有较高敏感性。结论 2型糖尿病患者泌尿系感染发病率高,病原菌多为条件致病菌,应及时做尿培养,根据药敏结果选用抗菌药物。

Abstract:OBJECTIVE To study drug-resistant bacteria in type 2 diabetes mellitus with urinary tract infections.METHODS By using Anhui stars HX-21 bacteria appraisal susceptibility analyzer,there were 218 patients with type 2 diabetes and urinary tract infection in hospital and clinic from Jan.2007 to Mar.2010,pathogens were identified and drug susceptibility was analyzed.RESULTS In 218 strains of pathogenic bacteria,mainly with escherichia coli(50.46%),followed by klebsiella pneumoniae(11.47%),pseudomonas aeruginosa(9.17%),staphylococcus aureus(8.26%),dung bowel coccus(7.34%),fungi(5.5%),bacillus proteus(4.13%),coagulase-negative staphylococcus aureus(3.67%).The resisdence of gram-negative bacteria to Cefoperazone sodium/Sulbactam sodium,Imipenem and Piperacillin/Tazobactam was low,but gram-positive cocci had a high sensitivity to Vancomycin.CONCLUSION In patients with type 2 diabetes,there is a high incidence of urinary tract infection with many conditional pathogenic bacteria,it should do urine culture,and choose antimicrobial agents according to antimicrobial susceptibility test results.
