托吡酯治疗儿童癫痫疗效及不良反应的临床研究

目的探讨托吡酯治疗儿童癫痫疗效及不良反应。方法研究对象为2008年11月～2010年3月我院接诊的癫痫患儿198例,给予托吡酯口服治疗,将0.5～1mg/kg.d定为初始剂量,每周加量1次(每次加药量同初始剂量),加至4mg/kg.d。随访1年,观察其疗效及不良反应情况。结果经1年随访发现,本组198例患儿中完全控制145例(占73.3%),好转48例(占24.2%),无效5例(占2.5%),总有效率达到97.5%;本组198例采用托吡酯进行治疗的癫痫患儿中,23例出现食欲不振,18例出现注意力受损,22例出现体重减轻,24例出现低热,27例出现出汗少,19例出现血尿,3例出现焦虑,5例出现遗忘,1例出现共济失调,3例出现恶心,2例出现意识模糊。结论通过本研究结果证实了在儿童癫痫的治疗中托吡酯疗效好,且不良反应发生率较低,是儿童癫痫的首选药物,值得临床上推广应用。

Study on the efficacy and adverse reaction of Topiramate in treatment of children with epilepsy

OBJECTIVE To study on the efficacy and adverse reaction of topiramate in treatment of children with epilepsy.METHODS From November 2008-March 2010,198 children with epilepsy were selected in our hospital.An oral therapy was performed to the children,0.5-1 mg/kg D was used as the initial dose,weekly added one dosage for one time(every time dosage with the initial dose),and added to 4 mg/kg D.They were followed up for 1 years to observe its efficacy and adverse reaction.RESULTS After 1 years of follow-up for 198 cases,145 cases(73.3%)could completely control,48 cases were improved(24.2%),5 cases were invalid(2.5%).The total efficiency was 97.5%,and 198 children with epilepsy were treated with topiramate,23 cases were loss of appetite,18 cases showed impaired attention,22 cases showed weight loss,24 cases showed low heat,27 cases appeared sweat,19 cases appeared hematuria,3 cases showed anxiety,5 cases appeared forgetting,1 cases appeared ataxia,3 cases showed nausea,and 2 cases showed consciousness.CONCLUSION The results showed the treatment of children with epilepsy could gain good curative effect,and there was low incidence of adverse reaction.Topiramate is the preferred option for the treatment of epilepsy,which is worthy of clinical application.
